Morris Hospital CARE COORDINATOR in Morris, Illinois

  • DESCRIPTION OF POSITION

  • The Accountable Care Organization (ACO) nurse coordinates team based care to provide, develop, implement, and manage programs set by Centers for Medicare & Medicaid Services (CMS) and our ACO consortium. These services deliver evidence-based practice that are provided to individuals, families, and/or their communities through effective partnerships with patients, their caregivers, and their physician to achieve high quality care that is patient/family centered.

  • QUALIFICATIONS

  • Professional/Educational

  • BSN required, MSN or APN preferred.

  • 3-5 years' experience in a clinical or community health setting.

  • Previous Care Coordination and/or Case Management experience preferred.

  • Previous experience with health IT systems and data reports preferred.

  • Previous experience with mobilizing community resources, navigating through the healthcare continuum and working with disparate populations preferred.

  • Ability to speak a relevant second language preferred.

  • DCFS Mandated Reporter Certificate of Completion required.

  • Demonstrates evidence of essential leadership, communication, education and counseling skills.

  • Proficient in communication technologies (e-mail, cell phone, etc).

  • Effective organizational skills, demonstrates ability to maintain accurate notes and records.

  • Demonstrates professional, appropriate, effective written, verbal and nonverbal communication skills.

  • Demonstrates a positive attitude and respectful, professional customer service.

    Actual pay will be determined by qualifications, experience, and internal equity.
