SUMMARY HCMC has a current opening for a Staff Nurse in Care Coordination. This position is a 0.5 FTE (5 shifts/ pay period), 4 hr/flex 40, Day Shift The current vacancy is for an RN Care Coordinator who will visit members in their homes to complete face-to-face assessments and develop a care plan for our SNBC (Special needs, Basic Care) Medica members. The position will primarily cover Hennepin County but there may be some cross-over. Once trained, the role will work remote out of the MVNA office. Mileage Reimbursement. RESPONSIBILITIES Within the context of the Hennepin County Medical Center "Centered Around You" Patient & Family Centered Care practice model, and under general supervision, the registered nurse provides and coordinates nursing care by: Protecting, promoting wellness and optimizing health Triaging patients by assessing severity of symptoms and referral to appropriate setting or level of care Alleviating suffering through nursing interventions and prescribed treatments Partnering with the patient, their family and significant others Documenting interventions and an individualized plan of care Educating patient on their disease process Assuring continuity of care Educating patient and family in preventative care which promotes health and well being QUALIFICATIONS Minimum Qualifications: 1 year of current professional nursing (RN) experience Graduation from an accredited school of nursing Possession of a valid license as a Registered Nurse issued by the State of Minnesota. Bachelor's Degree in Nursing required Public Health Nurse (PHN) certification required to obtain prior to start date in role. Valid driver's license and regular access to an insured automobile Graduation from an accredited school of nursing Possession of a valid license as a Registered Nurse issued by the State of Minnesota Preferred Qualifications: Public health professional nursing (RN) experience Bilingual language skill You've made the right choice in considering Hennepin Healthcare for your employment.
