Minnesota Visiting Nurse Agency Staff Nurse - Addiction Medicine Clinic in Minneapolis, Minnesota
*SUMMARY:* We are currently seeking aStaff Nurseto join our Addiction Medicine Clinic.
This position is a 0.6 FTE (6 shifts/ pay period), 8-hour shifts, Day/ Evening, Every Other Weekend.
* _RESPONSIBILITIES*_ Within the context of the Hennepin Healthcare System Relationship-Based Care practice model, and under general supervision, the registered nurse provides and coordinates nursing care by:
- Protecting, promoting wellness and optimizing health
- Triaging patients by assessing severity of symptoms and referral to appropriate setting or level of care
- Alleviating suffering through the treatment of the human response
- Partnering with the patient, their family and significant others
- Documenting interventions and an individualized plan of care
- Educating patient on their disease process
- Assuring continuity of care
- Educating patient and family in preventative care which promotes health and well being
Minimum Qualifications: * Graduation from an accredited school of nursing * Possession of a valid license as a Registered Nurse issued by the State of Minnesota
Preferred Qualifications: * Experience with Addiction Medicine * Care coordination experience * Certification in area of clinical specialty * BSN Preferred. Bachelor of Nursing degree is strongly encouraged. Advancement in academic education will be discussed as part of individual professional development during the annual performance review. // /If you are not currently in an Ambulatory department, you would be subject to the Ambulatory pay scale./
