This announcement will be used to fill a full-time Registered Nurse Senior position (MDS Coordinator). This position is responsible for the utilization and timely, accurate completion of the resident assessment instrument/MDS and care planning in accordance with federal, state, and Veterans Administration regulations as well as Medicare and managed care guidelines for coverage, billing, and reimbursement. Position also acts as a resource to the Interdisciplinary Team in the assessment and resident care planning process to assure the delivery of quality care.

Minimum Qualifications

To facilitate proper crediting, please ensure that your resume/application clearly describes your experience in the areas listed below and indicates the beginning and ending month and year for each job held. Only applicants whose resumes clearly demonstrate fulfillment of each of these minimum qualifications will be considered for this position.

  • Licensed in Minnesota as a Registered Nurse, in good standing.

  • Two years of experience as a Registered Nurse in a long-term health care setting with primary responsibility for assessing nursing and health needs.

  • Experience with documenting and writing reports.

  • Ability to communicate effectively.

  • Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.

Preferred Qualifications

  • Three years of experience as a Registered Nurse working in a Skilled Nursing facility with primary responsibility for coordinating and assessing plan of care.

  • Experience working in close cooperation with an interdisciplinary team.

  • Experience leading or supervising professional-level nursing staff.

Additional Requirements

  • Minnesota Department of Human Services (DHS) background check may be conducted on the top finalist(s) for the position(s). When conducted, a finalist must pass the DHS background check.

  • A Tuberculosis (Mantoux) screening (or chest x-ray) is required prior to employment.

  • Employment reference checks will be conducted on all finalists. This may include a review of documentation related to job performance. It also includes contact with the applicant's former employer(s).
