This is a Full Time 1.0 position that is allocated to provide services to patients and families on our inpatient general medicine units, Emergency Department, and Intensive Care Units. This position will also be trained in our different pediatric transplant specialties including Kidney, Liver and Heart as well as dialysis to provide cross coverage to those areas when needed.
The caseload is comprised of a mix of some short and long stay patients where the Clinical Social Worker may assist with needs that range from basic needs, education and advocacy for community resources including waivered programs, assist with referrals to county/state financial/insurance programs, coordination with school liaisons, support with matters relating to child protective services, mental health assessment/support/program referrals, social admissions, critically ill children where families may be facing a new diagnosis or perhaps goals of care or end of life decisions. The role of the pediatric clinical social worker is also considered the primary mental health provider on the interdisciplinary team and the Social Worker will support the patient and family’s coping and emotional needs, promote adaptation to illness and treatment, help families navigate through the medical system, bridging the connections between the family and the medical team, provide counseling to individuals, siblings, couples and families and help with problem solving, advocacy and communication about the patient’s plan of care.
There is an every 6th weekend rotation with two weekdays off in that pay period. Department hours are 8a-430p and we are onsite and patient facing. This position offers individual and group Clinical Supervision if needed.

Role Duties:
Support the patient and family’s coping and emotional needs.
Promote adaptation to illness and treatment
Help families navigate through the medical system, bridging the connections between the family and the nephrology interdisciplinary team.
Provide counseling to individuals, siblings, couples, and families
Help with problem solving, advocacy and communication about the patient’s plan of care
Help with school issues (arranging for home and hospital teaching, advocating for IEP and 504 educational plans, and guidance with school re-entry)
Help identify hospital and community-based resources: emotional, supportive and/or financial
Qualifications
LGSW/LICSW
Previous healthcare experience preferred.
Clinical supervision is provided.
