Position Summary The Clinician Referral Liaison serves as a primary point of contact and key interface between Children's Cardiology Institute and community and referring clinicians. The liaison will facilitate engagement and linkage of the Institute's clinicians and services to support communication and transitions of care for referred children and their families. The referral liaison will establish and maintain a database identifying referral clinician preferences regarding type, method, and frequency of communications. In addition, the referral liaison partners with Children's Physician Outreach and Children's Physician Access to deliver a seamless, consistent, and integrated referral process which honors the referring clinician service experience. The clinician referral liaison may respond to logistical needs of the family relayed by the referring clinicians prior to or in preparation for a planned visit to the Institute. License/Certification/Registration required? * Current Minnesota drivers license required. Education: * Two (2) years college course work or post-secondary education required in a health related field. Experience: * 3+ years experience in a position that incorporates customer service and organizing own work/being self-directed. * Direct experience working with pediatric sub specialty and referring and / or community clinicians preferred. * Demonstrated familiarity with pace and work flows of clinicians in inpatient and/or outpatient clinical environments. * Utilized medical terminology in previous work environments. * Worked directly with pediatric sub specialty and referring and/or community physicians is preferred. * Used multiple software applications, including electronic medical records. * Delivered customer service in a healthcare setting. Knowledge/Skills/Abilities: * Knowledge and familiarity with hospital inpatient and outpatient program and services. * Ability to solve problems and identify solutions. * Ability to communicate effectively with key stakeholders including referring clinicians, clinic practice managers/ office staff, Institute clinicians, and team members. * Ability to capture and organize information in a retrievable manner. * Ability to work independently and as a team member. * Requires ability to take initiative and meet objectives. * Requires being self-directed, highly motivated, and results driven. * Demonstrated customer service and organizational skills. * Ability to prioritize. * Ability to work collaboratively. * Requires excellent written, verbal, and interpersonal communication skills. * Active listening skills. * Requires ability to exhibit professional diplomacy. We carefully consider a wide range of factors inc uding but not limited to market indicators for the specific role, the skills, education, training, credentials and experience of the candidate, and organizational needs.
