Southwest General Health Center ONCOLOGY CLINICAL PHARMACIST in Middleburg Heights, Ohio
The Oncology Clinical Pharmacist provides expert patient-specific pharmacotherapy and pharmaceutical care to our infusion center patients and inpatients receiving chemotherapy. He/she serves as a resource to other health care professionals regarding clinical use of medications, maintaining cost effective use of formulary agents. The Oncology Clinical Pharmacist performs dispensing functions in our infusion center pharmacy, reviewing and verifying medication orders. He/she preforms therapeutic drug class reviews and drug utilization evaluations.
Graduate of accredited school of Pharmacy, Doctor of Pharmacy degree (preferred) from an ACPE-accredited College of Pharmacy or completion of Foreign Pharmacy Graduation Equivalence Exam.
Required length and type of experience:
Minimum of 5 years in an oncology environment with a graduate pharmacy degree or minimum one year experience in an oncology environment with a Doctor of Pharmacy degree
Required licensure, certification or registry:
Registered pharmacist, licensed by the Ohio Board of Pharmacy
BCOP (Board Certified Oncology Pharmacist) preferred
PGY-1/PGY-2 residency preferred
