Unit Summary

The Vanderbilt Health Affiliated Network is a collaborative alliance of physicians, health systems, and employers driving a new level of clinical innovation and teamwork to enhance patient care, contain costs and improve the health of communities in Tennessee and surrounding states (including portions of Kentucky, Eastern Arkansas, Northern Mississippi and the western portion of Virginia). The network includes more than 5,000 clinicians, 60 hospitals, 12 health systems and hundreds of physician practices and clinics who work together to strengthen communities and improve the quality of life across the Southeast through better health.

VUMC provides the professional expertise, legal framework, IT infrastructure and other resources to enable clinicians, hospitals and health systems to engage and succeed in population health efforts that improve the experience, cost and delivery of care. Your Role :

Provides psychosocial assessment of and brief targeted intervention for social, emotional and environmental issues that impact client/family's ability to optimally benefit from care. Actively collaborates with team to meet patient care management and system improvement goals. Represents the Social Work department on institutional and community committees/task forces. Provides leadership for department committees/workgroups. Contributes to the plan of care with knowledge of population specific needs as it relates to the identification of patient psychosocial needs.

Master's Degree (or equivalent experience) and < 1 year relevant experience

Licensure, Certification, and/or Registration (LCR):

Additional Qualification Information:

  • Must hold a current Tennessee Social Work License. Candidates who have recently graduated from an accredited Graduate School of Social Work or have moved to TN with another State's Master Social Work license are allowed a provision to obtain licensure from the TN State Board within six (6) months of hire date.
