Trinity Health Hospice Admission Nurse - Hospice Home Care - Full Time - Days/Evenings in Mason City, Iowa

Position Purpose:

Hospice Registered Nurse primarily responsible for admission assessments. This individual must have strong critical thinking skills, problem-solving skills, assessment skills, and the ability to develop a personalized plan of care involving the patient/family.

What you will Do:

  • The Admission RN is proficient in the admission assessment and has knowledge / accuracy in completing the admission assessment.

  • The Admission RN is fully responsible for performing the patient’s comprehensive assessment, development of an individualized plan of care, goals, and collaboration with the attending physician to assure orders accurately reflect the patient’s medications, treatments, activity, etc.

  • Accurately assesses and completes the admission assessment for hospice patients at admission and at other time points as needed.

  • Assesses patient safety and needs while determining the agency’s ability to meet those needs in the facility or home care setting.

  • Initiates and maintains communication regarding the patient’s care/needs with the patient/family, physician, clinical leader, case manager, facility staff, and other disciplines as appropriate.

  • Delivers nursing care to patients in their facility or home setting, utilizing appropriate available resources.

  • Involves the patient / caregiver or facility staff in developing and carrying out the plan of care.

  • Teaches and demonstrates care to the patient / caregiver or facility staff in the facility or home setting.

  • Evaluates and documents the patient’s physical and emotional reactions to treatments, care, and medications in the patient record.

Hours/Schedule:

Full Time 72hrs pay period with 12hr shifts. Will cover holidays that fall on scheduled days. Call is shared within team. May trade shifts with RNs working other days of the week.

Minimum Qualifications:

  • Current RN license is required.

  • BSN preferred.

  • Hospice and Palliative Care Certification preferred.

  • Must have demonstrated nursing competence in a health care setting for two years.

  • Caring for terminally ill patients/families, Hospice or home care nursing experience is preferred.
