Position Summary:
Manages the department’s activities and resources to drive quality outcomes.

ESSENTIAL FUNCTIONS:
-Review Electronic Medical Record (EMR) and payor registries to identify high risk patients. -Analyze clinical data and identifies Hierarchical Condition Category (HCC) gaps. -Query/communicate with provider/care manager to capture accurate Hierarchical Condition Category (HCC). -Assist in the evaluation of data collected to assure accuracy and reliability. Prepares reports and performance reporting of quality data. -Oversees quality reporting to the insurance carriers.
Minimum Qualifications:
Minimum Education Associate's Degree in Nursing - Required

Required Skills Registered Nurse in state of Michigan Basic Life Support (BLS)
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
