M Health Fairview has an immediate opening for an LPN to join our team at the Podiatry Clinic located in Maplewood MN. Join M Health Fairview, the largest healthcare system in the state! The M Health Fairview Podiatry Clinic is a growing and collaborative clinic.
This is a 1.0 FTE (80 hours per two week pay period), benefits-eligible opening, no weekends.
This position will split time between the Maplewood, Wyoming Lakes, Woodbury (Birch Center), and University Fulton Vascular Clinic locations.

Responsibilities Job Description
The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, or physician/practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines.
Collects data for patient assessments.
Involves the patient, their support network, and other health team members in collecting relevant data.
Reports unusual data or changes in patient's condition to the RN, or physician/authorized practitioner in a timely manner.
Delivers care as delegated by the RN or physician/authorized practitioner to assigned patient(s) population, or individual patient.
Follows the patient’s plan of care.
Prioritizes work based on patient acuity, available resources, and patient/support network preferences, in collaboration with other care team members
Performs high and low risk technical procedures, including medications, defined by the clinical setting.
Recognizes when different approaches may be needed based on age or cultural differences.
Implements assigned aspects of the patient's teaching plan.
Effectively communicates compliance and concerns to delegating RN or authorized practitioner.
Takes action to assure continuity of care for all assigned patients.
Responds appropriately to emergent situations.
Communicates successfully with all members of the health care team.
- Communicates verbal and written information in a clear, concise, accurate, and timely manner.
Qualifications
Required
Vocational/Technical Training/ Associates Degree
Current MN LPN License
Preferred
One to three years
Previous ambulatory Care experience.
Current BLS Certification
