Centra Wellness Network Access Intake Case Manager in Manistee, Michigan
The Case Manager works with a specific specialty population, i.e. adults living with Severe Mental Illness (SMI), children and youth living with Severe Emotional Disturbances (SED), and/or adult or children living with Intellectual/Developmental Disabilities (I/DD), and individuals living with co-occurring substance use disorder.
Works with the client and his/her chosen support network in developing treatment and recovery plans that promote individual choice, the use of community resources, and formal and informal supports in pursuit of the individual's desired future. Assists the client in the assignment of available agency resources to the plan and monitor those supports and services according to the plan. Services and supports are based on the principles of recovery and self-determination.
The Case Manager works under the clinical supervision of the Clinical Services Supervisor.
