I. General Description of Responsibilities:
The Director of Support Services ensures the quality of the assigned programs for his/her service cluster by monitoring the services and programs for all the clients in accordance with Journey Found policies and procedures. This position reports directly to the Chief Operating Officer. Specific responsibilities in the areas of supervision and management, training, evaluation, planning and development are as follows:
II.Supervision and Management:
A. Directly supervises assigned Program Managers to ensure that individuals served are provided with an intensively therapeutic program designed to enhance community acceptability and to increase the individuals’ level of independence in the areas of self-care, community integration, as well as all aspects of activities of daily living.
B. Conducts monthly Administrative Site Reviews and provides program review data across programs that meet an overall effectiveness rating of 80% or better. In addition, conducts observations in all areas to determine the rate and quality of staff interactions, use of incidental teaching opportunities, distribution of attention across groups/individuals, rate and quality of community contact, and consumer appearance on a monthly basis. Provides Program Managers with feedback based upon the above assessments on a monthly basis.
C. Facilitates the team review and the approval of semi annual summaries of completed programming for each individual served and ensures that documentation is forwarded to the Department of Developmental Services in accordance within established timelines.
D. Ensures the safety and welfare of all individuals served by directing self-preservation training, in conjunction with Program Managers, and informs the Chief Operating Officer of any outstanding concerns in a timely fashion.
E. Ensures that communication between Journey Found and the various members of the interdisciplinary team exists to obtain appropriate services for the individual.
F. Monitors staffing patterns, safety data, DCER and consumer spending.
G. Meets as needed with Chief Operating Officer.
H. Meets with Program Managers individually formally once per month with weekly contact between meetings, to provide brief descriptive/corrective feedback and suggestions as needed, makes on-the-spot decisions about routine problems, and notifies the Chief Operating Officer about problems relating to policy and procedures, physical environment, ethics, and community relations.
I.Participates in Program Manager meetings, general staff meetings as scheduled, admin team meetings, clinical meetings, and review meetings as scheduled for consumers.
J. Facilitates development and implementation of consumer IP process and implementation to ensure that treatment procedures are conducted as specified in each individual’s plan.
K. Meets as needed with the nursing team to ensure the quality of health care of the individuals served.
L. Initiates and documents contact with families or guardians no less than monthly to assess their satisfaction with the services provided.
M. Ensures that the Program Managers contact families/guardians on a weekly basis or, on a time frame agreed to by the family or guardians.
N.Ensures the preservation of the rights of each individual served and his or her family by maintaining confidentiality, employing restrictive procedures only when specified in an individual’s treatment plan and following the ethical standards of Journey Found.
O. Cooperates with special requests made the Chief Operating Officer, Chief Executive Officer or other Journey Found Directors.
P.Assists in review of Incident Reports according to DDS and Journey Found policies
Q. Conducts annual performance evaluations for Program Managers according to Journey Found Policies. Ensures that Program Managers conduct annual performance evaluations for all staff.
R. Develops and maintains a good rapport with funding agencies, keeping them informed of critical concerns in a timely fashion in accordance with regulations promulgated by the Department of Developmental Disabilities, other funders and Journey Found.
S. Ensures that program is appropriately furnished to meet the needs and desires of the individuals served and that the program is clean, in good repair and is safe from hazardous conditions. Will ensure that the vehicles used to transport individuals served are clean and in good repair
T.Provide 24/7 on call to programs assigned all weekdays and on weekends as scheduled.
Adheres to the mission, vision and values of Journey Foundand conducts him/herself within standards of professional conduct of his/her program, division and the organization. Adheres to the Journey Found policy & procedure which protects the privacy and security of consumer health information. Maintains the security of electronic media and returns to his/her supervisor when appropriate.
ESSENTIAL PHYSICAL REQUIREMENTS:
Essential functions of this position will require the ability to communicate orally and in writing to a wide variety of audiences. The ability to move independently throughout a wide range of environments, some of which may not be wheelchair accessible, is required. The ability to physically participate in crisis prevention training is required and to implement the trained procedures in a crisis situation with individuals served. The ability to lift inanimate objects such as files and records is needed as well as the ability to assist in the lifting of individuals served. Independent transportation to locations that may not be served by public transportation is required. The work schedule and hours will be fixed however; the schedule will reflect the needs of the program. There will be times when training or other special needs will occur outside of the fixed schedule. The position will require the individual to have a valid CT. Driver's License and transporting of individuals served as safe and appropriate will be an on-going responsibility.
