14000BRDepartment Name:HSP Perinatal Clinic Location:Meriter Hospital Job Description: The Telehealth Registered Nurse will be responsible for the oversight of the clinical portion of the Staying Healthy After Childbirth, a Postpartum telehealth program for remote patient monitoring. This position will be responsible for patient education on the use of the equipment, as well as daily monitoring of the patient physiologic parameters through the Honeywell Lifestream web application. This position will assist the primary OB provider in the management of hypertension through consistent remote monitoring to decrease the potential of postpartum readmissions due to hypertension.

Job Duties:
Provide telehealth nursing care to participants enrolled into Staying Healthy After Childbirth program.
Maintain patient records, including but not limited to: documentation of vital signs, changes in medications used by the study participant, and other side effects of hypertension.
Perform independent, remote Nurse Visit, complete assessment, and document in EMR.
Notification of primary care provider for physiologic changes outside set parameters.
Complete reminder calls to participants who have delayed entry of physiologic data.
Support and collaborate with research studies related to this program and other maternal initiatives
Creation and maintenance of reports capturing pre-identified data.
Qualifications:
Nursing degree
1 year of nursing experience
Valid driver’s license when driving any vehicle for work-related reasons
State of WI Nursing License
Use of usual and customary equipment used to perform essential functions of the position
Ability to multitask and prioritize projects based upon deadlines and interdisciplinary collaboration
