UW HOSPITAL & CLINICS RN Transitional Care Case Manager in MADISON, Wisconsin

JOB REQUIREMENTS: RN Transitional Care Case Manager - 36348 DESCRIPTION/RESPONSIBILITIES: 100% FTE, Monday - Friday, 7:30am - 4:00pm. No weekends or holidays. Hours may vary based on the operational needs of the department. You will be working at University Hospital and East Madison Hospital in Madison, WI. We are seeking a Registered Nurse (RN) Transitional Care Case Manager to: * Bridge inpatient to ambulatory care for medical, surgical, and trauma populations. * Coordinate with interdisciplinary teams to ensure remarkable healthcare. * Use clinical expertise to empower patients during post-acute hospitalization recovery. * Be an integral part of a dynamic, innovative, and fun team.
