UW HOSPITAL & CLINICS RN Navigator, Pulmonary - 32571 in MADISON, Wisconsin

JOB REQUIREMENTS: RN Navigator, Pulmonary - 32571 DESCRIPTION/RESPONSIBILITIES: 80% FTE, Monday - Friday, between the hours of 8:00am - 5:00pm. Hours may vary based on the operational needs of the department. You will be working at the University Hospital in Madison, WI. We are seeking an RN Navigator to: * Work in collaboration with numerous multidisciplinary teams to help navigate patients and families through Pulmonary care. * Be a conduit of information between inpatient and outpatient services and pre- and post-surgical care for patients and families. * Participate in continuous quality improvement initiatives. * Work closely with Pulmonary providers, carry out coordinated plan of care for patients, and be highly organized and great at multi-tasking. Preferred - Masters degree in Nursing, Social Work, Public Health, Hospital Administration or related field. Work Experience : Minimum - Three (3) years of relevant clinical nursing experience. Preferred - One (1) year relevant experience in the specific patient population they are supporting. Licenses and Certifications : Minimum - Registered Nurse license in the State of Wisconsin and Basic Life Support/CPR.
