13965BRDepartment Name:HSP Performance Improvement Location:McConnell Hall Job Description: The Patient Safety Officer (PSO) has direct responsibility for leading the UnityPoint Health-Meriter Patient Safety Program as Chairperson of the Patient Safety Committee to implementation of patient safety related action plans and initiatives outlined in the Meriter’s Quality Assessment and Performance Improvement Plan and as directed by the Quality Management Oversight Committee. The PSO acts as a resource to the hospital, UnityPoint Health-Meriter Clinics and healthcare community on issues related to patient safety through coaching, teaching, and collaboration.

Responsibilities:

  • Serves as Chairperson of the Patient Safety Committee assuring implementation of patient safety action plans and appropriate oversight of safety trends.

  • Facilitates groups of key stakeholders or teams focused on understanding of patient safety issues, implementation of improvement strategies and root cause analyses of events and near misses. Partner with Infection Prevention program on related issues.

  • Is content expert on patient safety topics for Meriter employees, physicians, students, and members of Meriter governing boards.

  • Coordinates the collection, aggregation, analysis and reporting of patient safety related data which includes oversight of the patient incident reporting system and related functions.

  • Is knowledgeable in improvement science. Utilizes patient safety data to develop focused improvement plans, facilitate implementation of action plans, and monitor measures of success.

  • Provides patient safety related education to key stakeholders.

  • Oversees patient safety-related accreditation and regulatory standard compliance.

  • Professional development and leadership responsibilities for direct reports (e.g., performance evaluations, time management/productivity, assignment of responsibilities).

  • Communicate key quality performance results, corrective actions, and organizational impact to stakeholders.

  • Serves at the UnityPoint Health- Meriter liaison to UW Health and UnityPoint Health in development and alignment of Patient Safety activities or delegates representation on related committees/workgroups to ensure that Meriter stakeholders are represented in decision making.

  • Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.

  • Demonstrates ability to meet business needs of department with regular, reliable attendance.

  • Employee maintains current licenses and/or certifications required for the position.

  • Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.

  • Completes all annual education and competency requirements within the calendar year.

  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Requirements:

  • Master’s degree in a health care profession such as health care administration, MBA, nursing, or other

  • 5 years of progressively responsible experience in areas related to clinical care quality, patient safety, management of staff in a clinical setting, performance improvement or process redesign

  • Experience in management of complex projects or change initiatives

  • Experience as a facilitator of process improvement teams

  • Previous experience in education and training of both managers and staff

  • Valid driver’s license when driving any vehicle for work-related reasons.

  • Current RN or other professional licensure as applicable

  • Strong leadership skills

  • Excellent verbal and written communication skills

  • Effective organizational and time management skills

  • Proficiency in Microsoft programs (Word, Excel, PowerPoint)

  • Ability to conduct basic data analysis

  • Use of usual and customary equipment used to perform essential functions of the position.

    Preferred:

  • Additional coursework in patient safety, clinical risk management, quality management and / or health care administration

  • 3 years management experience in a health care setting

  • Certification through a national organization such as Certified Professional in Healthcare Quality(CPHQ), American Society for Healthcare Risk Management (ASHRM) or the American Society of Professionals in Patient Safety (ASPPS)

  • Presentation skills as demonstrated through public speaking and / or teaching situations
