COMMUNITY LIVING ALLIANCE INC Community Support Therapist in MADISON, Wisconsin

JOB REQUIREMENTS: The Community Support Therapist (CST) works with Community Living Alliance (CLA) clients to support them to achieve their highest possible level of independent functioning, stability, and facilitate recovery. This position conducts client assessment and care planning, coordinates and facilitates services and supports continuity of care upon discharge. This position is responsible for providing direct clinical therapy services to clients who are experiencing mental health, co-occurring conditions, and substance abuse recovery. Services will be provided in the client\'s home, through telehealth, or at CLA\'s office. The CST may serve as a Mental Health Professional (MHP) and/or Substance Abuse Professional (SAP) for CCS clients and will provide consultation and/or supervision for CLA CWS team members, as needed. Provide ongoing counseling and support to assigned clients. Create and manage a treatment and assessment plan. Collaborate with client support teams to address unmet long term care needs, when appropriate. These services may be provided in the client\'s homes or CLA offices. Provide back up to team-assigned clients. Provide MHP consultation with team for CCS clients and/or consultation and education to CLA team members and providers working with client. Provide necessary support for individuals working toward recovery using a supportive, strengths-based, culturally humble approach to treatment. Maintain documentation within CLA and county/regulatory information systems including but not limited to case notes, goal development, and client service/goal related activities. Participate/collaborate in client meetings and service coordination referrals. Utilize effective therapeutic strategies to engage client, enhance motivation, and encourage meaningful, collaborative and strengths-based goal planning and progress. Provide conflict resolution/mediation, lifestyle coaching and support, and advocacy with individuals. Assist the client with identifying and establishing natural and professional supports in the community.
