Down East Community Hospital CCPP RN in Machias, Maine

Job Summary & Scope:

The Provider Practice RN works in partnership with the clinic providers and other members of the healthcare team to provide care to the entire patient population. The RN focuses on providing critical thinking, assessment, screening, healthy lifestyle support, education, and chronic disease management with a goal of improving health outcomes and facilitating access to services.

Minimum Requirements:

  • Graduate of accredited school of nursing.

  • Active RN License in the State of Maine; or currently licensed RN from Compact state and must become licensed in the State of Maine within 90-120 days.

  • Proficient in the use of electronic medical records, Microsoft Office, and telephone communication.

  • Current BLS or certification prior to start date.

  • Maintains all licensure and certification requirements.

  • Two (2) years of clinical experience preferred.

    Essential Job Functions:

  • Demonstrates critical thinking, professionalism, and competency in the RN Role.

  • Clinical oversight and support of Licensed Practical Nurse (LPN) and Medical Assistants (MA) in the clinic(s).

  • Collaborates effectively with all members of the care team to ensure excellent patient care.

  • Provides telephone and in-person triage and patient care according to established practice protocol and scope of practice.

  • Engages patients in education regarding chronic illnesses such as diabetes, chronic obstructive pulmonary disease, hypertension, cardiac disease, cancer, and chronic pain.

  • Provides telephone care, follow-up and consultation for transitional care management, medication management, INR management, et cetera.

  • Determines the need for and initiates consultations with other services and healthcare providers and coordinates services and care with the patient to ensure continuity and follow-up.

  • Documents all care and pertinent information in the patient’s medical record accurately and timely.

  • Participates in program development and quality initiatives related to improving patient care, health, and safety.

  • Stays informed of necessary information by attending required meetings and reading correspondence such as emails, memos, newsletters, and policies in a timely manner.

  • Demonstrates a clear understanding of, and commitment to, DECH/CCH mission, vision, and values.

  • Supports and advocates for Evidence Based Medicine and Just Culture.

  • Other duties as assigned by Manager.
