Responsibilities

Position Summary:

Manages the day to day operations of the Pharmacy. Responsible for ensuring all distributive functions are completed in a safe and efficient manner while helping to ensure regulatory compliance. Responsible for providing clinical pharmacy services to meet the needs of patients and support the mission of the Department of Pharmacy, Hospital, and Health System. Provides comprehensive pharmacy care through protocol/guideline management as well as individualizes drug therapies to promote safe, efficacious and cost-effective patient care. Serves as a role model for pharmacy residents and fellows, as well as pharmacy students. He/she creates and contributes to the ongoing education and development of pharmacy personnel and the education of other health care practitioners. Displays professional, safe conduct in all interactions. Maintains positive working relationships with staff, other departments, physicians and peers. Displays conduct in support of CHI St. Luke’s Health Memorial Mission & Vision.

  12. Contributes to formulary management and Works with health care providers to ensure formulary compliance.

  13. Actively Participates in medical staff and service line meetings as requested, including Pharmacy & Therapeutics Committee, to meet patient care needs and Service Line objectives.

  14. Participates in Service Line initiatives as related to medications and the medication use process.

  15. Participates in projected cost savings/cost avoidance activities related to medication utilization and Provides routine reports on status.

  16. Participates in Strategic and Operational Planning activities and the development of the Pharmacy Department’s annual budget.

  17. Educates, mentors, and evaluates pharmacists, pharmacy residents, and/or pharmacy students to develop and improve skills necessary for providing pharmaceutical care.

  18. Educates other health care professionals regarding medications to promote optimal patient care via in-services, educational programs, face-to-face interactions, etc.

  19. Reviews, interprets, and evaluates all medication orders for patients in all age groups.

  20. Maintains patient medication profiles in an automated pharmacy information system.

  21. Consults with and advises medical, nursing, and other healthcare providers regarding the prescribing, dispensing, or administration of medications.

  22. Routinely monitors the patient’s total drug therapy for effectiveness, side effects, adverse reactions, toxicities, drug interactions, and incompatibilities.

  23. Serves as a pharmacy resource in cardiopulmonary emergencies.

  24. Conducts assigned target drug monitoring programs and concurrent drug use evaluation activities as required

  25. Performs and documents Pharmacist Intervention activities such as recommendations on drug product selection, dosage adjustments, and appropriate drug therapy.

  26. Obtains medication histories on selected patients and communicates pertinent information to the appropriate healthcare provider.

  27. Communicates with physicians, nurses, and other healthcare providers as necessary.

  28. Prepares and/or Verifies the appropriate preparation of medications to be dispensed using the correct technique, as required.

  29. Provide staffing coverage & assists with work schedules. Maintains time and attendance as required.

  30. Establish/maintain effective and respectful communication in all collaborative relationships.

  31. Assist in orienting and training staff and assists in assessing and/or documenting competency of staff within section.

  32. Represent CHI St. Luke’s Health Memorial in physician, community relations and vendors in a professional/accountable manner.

  33. Perform duties in accordance with that of hospital policies and procedures, and regulatory agency guidelines.

  34. Accountable for maintaining required certifications and ensuring same of staff.

  35. Performs call responsibilities in a professional and accountable fashion as required.

  36. Perform other duties as assigned.

Qualifications

Minimum Qualifications:

Education Level

Required:

Bachelor of Science in Pharmacy (B.S.) OR

Doctor of Pharmacy (Pharm D) degree

from a Board approved, ACPE Accredited College (pdf) of pharmacy program, OR

Graduated from a college of pharmacy program outside the U.S. and have obtained full certification from the Foreign Pharmacy Graduate Equivalency Committee (FPGEC)

Licensure/Certification

Required:

Licensed Pharmacist in the State of Texas through Texas State Board of Pharmacy (TSBP) &

Basic Life Support (BLS) certification (through an approved AHA program) &

*Completion of an ACPE approved 20 hour sterile products course (must be completed within 60 days of date of hire &/or transfer into position)

Experience

Preferred:

Two (2) Years of experience as a Clinical Pharmacist in an acute care setting.

Master’s degree in business administration or similar field of study

Completion of pharmacy administration residency

Completion of pharmacy (PGY-1) residency

Pay Range

$49.08 - $71.16 /hour
