CommonSpirit Health at Home Manager, Health at Home Navigator in Loveland, Ohio
Overview
The Health at Home Navigator (HHN) Manager provides support to Health at Home Agency Directors and Care Management Directors in our wholly owned and joint venture locations, managing HHN employees across multiple health systems nationwide. HHNs in markets with no assets or in MSA markets will report directly to the HHN Manager and have a dotted line reporting relationship to the Directors of Care Coordination.

Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.
Interviews, hires, and manages complex onboarding processes
Creates connections with HHNs through mentorships and national support.Facilitation HHN calls across all markets.
Supports local market directors and Directors of Care Management in implementing and evaluating the effectiveness of the HHN role.
Addresses opportunities and barriers based on experience across diverse markets.
Assists in creating and navigating reports lists in EMR
Facilitate support through mentorship and HHN group meetings
Create, update, and disseminate new educational materials, including orientation manuals.
Creates and monitors KPIs to demonstrate the HHN’s value to hospitals and leadership.
Drives the growth of the HHN program/role, creating and ensures the use of up-to-date data dashboards and scorecards.
Oversight of data distribution to appropriate stakeholders.
Communicates with the CSH analyst the needs of new HHN’s for data collection and solve for and mitigate data collection barriers.
Manages direct reports responsible for all HR processes
Assists with Contracts and A2’s
Onboarding with input from Hospital CC.
Supports the navigation of complex relationships with contractual labor and shared oversight to ensure the success and expansion of the Health at Home Navigator program.
Collaborates with the national Care Management team and other stakeholders to enhance processes for scalability and efficiency
Assists Health at Home leadership in developing and delivering presentations to educate Hospital Executive Leadership Teams (ELTs), care coordination teams, physicians, and Health at Home staff.
Supports evolution and expansion of the scope of the HHN to include PHSO, medical practices, SNF and IRF and others as directed by organizational leadership

Qualifications
Required Education: RN or MSW
Required Minimum Experience: 5 years as an RN or MSW
Preferred Education and/or Experience: Bachelor's degree preferred. Experience in HHN / HHAd
Home Health or Hospice leadership preferred
Pay Range
$36.66 - $53.16 /hour
