Amergis Public Health Nurse (PHN) in Los Angeles, California

The Public/Community Health Nurse (PHN) is responsible forthe mental and physical well-being of the community by ensuring the public’shealth through health promotion, disease prevention, treatment, and education.They can work in a variety of institutions such as the community, hospitals,clinics, or long-term care facilities while collaborating with stateregulators, doctors, and other healthcare professionals to keep the communitysafe and healthy.

Essential Duties and Responsibilities:

  • Assesses health care trends in communities

  • Collaborates with emergency response planning and trainingwhen applicable

  • Works with public health officials to help communitiesaccess and receive care

  • Provides immunizations and health screenings to all populationsin the community

  • Provides patient and community education

  • Monitors and delivers high-quality patient care

  • Records and analyzes medical data in various communities

  • Evaluates the health of patients and develops treatmentplans in collaboration with medical providers and multidisciplinary teammembers

  • Participates in the evaluation and documentation of patientsand care

  • Assists in developing, evaluating, and implementingprofessional development education

  • Refers patients to other providers as needed

  • Manages budgets of public health facilities if applicable

  • Reviews the work of assigned team members for thoroughness,soundness of recommendations, and compliance with the standards, policies,procedures, and regulations of the CDC as well as the County Department ofPublic Health

  • Performs other duties as assigned/necessary

Minimum Requirements:

  • Active Nursing Licensure in the state practicing

  • Two (2) years of public health or community experiencepreferred

  • Complies with all relevant professional standards ofpractice

  • Participation and completion of Amergis' Competency programwhen applicable

  • Current CPR if applicable

  • TB Questionnaire, PPD or chest x-ray if applicable

  • Current Health certificate (per contract or stateregulation)

  • Must meet all federal, state and local requirements

  • Successful completion of new hire training as applicable tojob site

  • Understand patient confidentiality and HIPAA requirements

  • Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required

  • Computer proficiency required

  • Must be at least 18 years of age
