Nurse Practitioner (NP)

Location: Los Angeles, CA

Compensation: up to $125,000 annually

Work Environment: Hybrid - primarily work from home with potential visits to patient homes, hospitals, facilities, and main offices.

Overview We are seeking a dedicated and skilled Nurse Practitioner to join our team. The NP will focus on high-risk patient engagement. Responsibilities include assessing healthcare needs, implementing patient action plans, coordinating care transitions, and providing disease education. The NP works within an interdisciplinary team to ensure quality outcomes for patients with chronic conditions, always involving the caregiver in the care management process.

Primary Responsibilities:

  • Develop and utilize alert-driven care pathway templates by condition and risk level.

  • Deploy remote patient monitoring for high-risk chronic conditions.

  • Conduct in-home or tele-health assessments as needed.

  • Respond in real-time to patient alerts and communicate with members and caregivers.

  • Coordinate patient care, including chronic disease management and interdisciplinary collaboration.

  • Conduct assessments and develop member action plans with patients and caregivers.

  • Engage patients in managing their health and refer to appropriate community resources.

  • Follow evidence-based guidelines to close gaps in care and facilitate use of in-network services.

  • Document care coordination activities in the electronic medical record system.

  • Participate in team-based rounds and ongoing program design.

  • Reassess member action plans post-discharge.

General Duties:

  • Collaborate with other business divisions to define and achieve business objectives.

  • Provide clinical expertise in assessment and event-based care management.

  • Create a productive, collaborative, safe, and inclusive work environment.

Qualifications:

  • Education: Active California state Registered Nursing license; BSN required.

  • Experience: At least 5 years of clinical experience, with 3+ years in care management, health plans, home health, or hospice.

  • Skills: Excellent communication, presentation, and proficiency in Microsoft Office applications.

  • Attributes: Strong relationship-building, good judgment, impeccable ethics, and a team player with a desire to grow in a fast-paced environment.
