Bon Secours Mercy Health Community Health Program Coordinator - Parish Nursing in Lorain, Ohio

Primary Function/General Purpose of Position

The Community Health Program Coordinator is responsible for planning, implementing, and evaluating community health programs aimed at improving the health and wellness of the community. The primary focus is on addressing the needs of underserved and vulnerable populations, including low-income families, ethnic minorities, elderly individuals, and those with chronic health conditions. The Coordinator works closely with the community health team, faith-based organizations, community members, healthcare providers, and local organizations to develop and sustain programs that enhance the overall health of these populations.

Essential Job Functions

  • Design, plan, and execute community health programs based on assessed needs and in alignment with the Community Health Needs Assessment and Community Health Improvement Plan.

  • Develop program goals, objectives, and timelines in collaboration with stakeholders.

  • Ensure programs are culturally relevant and accessible to diverse populations.

  • Foster strong relationships with community members, local organizations, and other stakeholders.

  • Conduct outreach activities to promote health programs and services.

  • Organize and facilitate community meetings, workshops, and health education sessions.

  • Conduct needs assessments to identify community health issues and priorities.

  • Develop and implement evaluation plans to measure program effectiveness and impact.

  • Collect and analyze data to inform program improvements and decision-making.

  • Coordinate with healthcare providers, government agencies, and non-profit organizations to enhance program reach and effectiveness.

  • Participate in local health coalitions and networks to share resources and best practices.

  • Advocate for policies and practices that improve community health outcomes.

  • Prepare reports and documentation for funders and stakeholders.

  • Develop educational materials and resources on various health topics.

  • Implement strategies to raise awareness about health issues and promote healthy behaviors.

  • Provide training and support to volunteers and department staff.

  • Prepare reports and presentations on program impact and health trends.

  • Participate in program evaluation and quality improvement initiatives.

  • Maintain patient and staff confidentiality according to hospital and department policies.

Education Qualifications

Bachelor’s degree in Public Health, Health Science, Social Work, or a related field is preferred.

Master's degree preferred.

Licensing/Certification

CPR certification is required within 30 days of hire.

Certified Health Education Specialist (CHES) preferred.

Work Experience

Minimum of 3-5 years of experience in community health, program coordination, or a related area.

Other Knowledge, Skills and Abilities (Required)

  • Strong knowledge of personal computers and advance proficiencies in word-processing, spreadsheets, and graphics software including Microsoft Word, Power Point, Excel, Access, audio visual equipment, etc.

  • Strong organizational and project management skills.

  • High attention to detail and numerical accuracy.

  • Ability to read, write, and perform basic arithmetic.

  • Possession of a personal automobile, current Ohio driver's license, and automobile insurance with $100,000/$300,000 liability required.

  • Ability to work evenings or weekends as needed.
