Eastern Ohio Corrections Center Case Manager in Lisbon, Ohio

Position: Case Manager Type: Treatment Reports to: Program Manager I PURPOSE Under the direct supervision of the Program Manager, the Case Manager is responsible for providing case management services to felony offenders committed to the Eastern Ohio Correction Center. Such services include the coordination of assessments, development and continued monitoring of structured treatment plans, and any other activities required in assisting and supporting offenders in gaining access to needed medical, psychological, social, educational, employment, or other services that are essential to meeting basic human needs. II ESSENTIAL FUNCTIONS & RESPONSIBILITIES a) Administer assessment instruments to offenders and interpret results to determine and/or identify specific treatment needs. b) Assist the offender in identifying and developing specific treatment goals and objectives. c) Meet face-to-face with offenders and discuss progress towards goals and objectives, treatment services, and/or other areas of identified need. d) Ensure that individual offenders are continuously supplied with the resources necessary to achieve goals and objectives. e) Explore all resources for necessary services and make referrals where appropriate and available to community agencies, organizations, and institutions. f) Act as an advocate for the offender to ensure that the proper services are being delivered according to the established treatment plan, and to ensure the continuation of such services until such time as all goals and objectives identified with the service are successfully achieved. g) Prepare and maintain treatment plans, reports, progress notes, periodic evaluations, and other written Data or information regarding an offender's individual treatment. h) Provide supervisors and other specified personnel with summary reports, evaluation reports, or other data when requested. i) Participate in treatment team meetings with other Case Managers and service providers to identify offender successes and failures, review cases, review and act upon disciplinary infractions or other behavioral issues, and make alterations to an offender's individualized treatment plan when necessary. j) Serve as a representative of the facility by displaying compassion, fairness, integrity, and professionalism in all interactions with other members of the Eastern Ohio Correction Center community and the general public. III OHER DUTUES & RESPONSIBILITIES a) Communicate the probation officers regarding an offender's progress within the program, revisions to the offender's treatment plan, special behavioral, disciplinary, or supervisory issues, and suggestions for the offender's after-care or post-release treatment activities. b) Assist program facilitators when necessary and/or required in delivery of program services. c) Provide individual counseling or crisis-intervention services when required or requested. d) Assist facility security staff when required or requested. e) Transport offenders to treatment-oriented functions and appointments within the community. 0 Collaborate and participate with other staff in designing, developing, and implementing new treatment programs and offender activities. g) Participate in relevant professional organizations and attend meetings, conferences, workshops, and seminars sponsored by professional organizations. h) Participate in facility committee meetings and other facility functions. i) Perform other incidental and related duties as required and assigned. IV EDUCATION & ABILITIES a) The Case Manager must meet the following minimum educational and experience requirements: Created 03/22/2022 i) A valid motor vehicle operator's license ii) A bachelor's degree from an accredited college or university in a related course of study (e.g. Behavioral Science, Social Work, Criminal Justice, etc.) OR i) A high school diploma
