The Children's Home of Wyoming ( CHOWC) Care Manager in Liberty, New York
Overview: The Children's Home of Wyoming is seeking Full-time Care Manager in Liberty.
The Care Manager must know the social service and health care systems and be able to build relationships with families and partners. Applicants must be comfortable making home visits to clients homes, working closely with schools, health providers, social services, community service providers, etc. Additional duties include completing assessments, facilitating team meetings, writing plans of care, documenting all activities, referring clients to needed services, etc.

Requirements; Physical Exam Reference Checks Drug Screening Class D NY State Valid License Background Check Bachelor's Degree 2 years Experience
Shift, Salary and Benefits: This is a full-time position, Monday-Sunday, 8:00 AM - 4:00 PM, $20.00 to $24.00 hourly. Benefits include; Health Insurance, Dental Insurance, Paid Vacation, Paid Sick Leave, Holidays, and 401K/Retirement.
