Universal Health Services Discharge Planner in LIBERTY HILL, Texas

Responsibilities

As the Discharge Planner, you'll play a critical role in facilitating timely and effective discharges for our patients. Under the supervision of our Director of Social Services, you'll work closely with our interdisciplinary team to ensure a seamless transition from our facility to the next level of care.

Your Impact:

  • Help patients and families navigate the discharge process and ensure a smooth transition

  • Collaborate with our interdisciplinary team to provide comprehensive care and support

  • Utilize our electronic medical records (EMR) system to track patient progress, document case management notes, and support individualized treatment plans

  • Foster open communication lines with our Director of Social Services, Program Managers, and other staff members to ensure seamless care coordination

    Qualifications

What You'll Bring:

  • Master's degree in a mental health field from an accredited college or university

  • A passion for working with patients and families and a commitment to delivering high-quality social work services

  • Experience with discharge planning, case management, and patient advocacy

  • Strong communication, interpersonal, and organizational skills

  • Ability to work collaboratively as part of a multidisciplinary team

  • Knowledge of external review organizations, payor sources, financial planning, and contracting (e.g. managed care, Medicare, Medicaid, Tricare)
