TheLicensed Clinical Social Workerin the Section ofGeriatric Psychiatryat Dartmouth-Hitchcock Medical Center will play a pivotal role in providing comprehensive assessments, care, and support to older adults with primary psychiatric diagnoses and cognitive disorders and their care partners. This position involves the opportunity for a unique blend of medical social work and psychotherapy, focusing on the complex needs of elderly individuals facing mental, physical, and cognitive health challenges. The ideal candidate will have a strong background in clinical social work and a deep commitment to enhancing the quality of life and care for older adults and their families.

We are seeking candidates who can bring the following to this important role:

  • Minimum of two years of experience in clinical social work, preferably in geriatric psychiatry or a related field.

  • Strong knowledge of psychiatric disorders commonly seen in older adults, including mood disorders, anxiety disorders, psychotic disorders, substance use disorders, and neurocognitive disorders (e.g., Alzheimer's disease, vascular dementia, etc).

  • Experience providing psychotherapy and counseling to individuals and groups, focusing on evidence-based practices and culturally competent care.

  • Excellent interpersonal and communication skills, with the ability to collaborate effectively with interdisciplinary teams and engage patients from diverse backgrounds.

  • Demonstrated ability to work independently, manage multiple priorities, and maintain professional boundaries in challenging clinical situations.

This full-time, 40-hour/week day-shift position is located on-site at our Lebanon, NH campus.We are offering a $7000 sign-on/retention bonus for this position. *The bonus is available for external hires only, and certain conditions apply.

Responsibilities

  • Conduct comprehensive psychosocial assessments of geriatric patients to evaluate their mental health needs, social support systems, and functional abilities.

  • Develop and implement individualized treatment plans in collaboration with a geriatric psychiatry interdisciplinary team, incorporating input from psychiatrists, nurse practitioners, nurses, and other healthcare professionals.

  • Provide evidence-based psychotherapy and counseling to geriatric patients, utilizing modalities such as cognitive-behavioral therapy (CBT), supportive therapy, problem-solving therapy, and mindfulness-based interventions.

  • Facilitate psychosocial treatment, education, and support for care partners and families tailored to the unique needs of geriatric patients, addressing topics such as coping skills, caregiver stress, communication strategies, advance care planning, long-term care planning, and grief.

  • Offer crisis intervention services and support to patients and their families

  • Advocate for patients' rights and access to appropriate mental health and supportive services, collaborating with community resources, facilities, and agencies as needed.

  • Conduct ongoing assessments of patients' treatment progress and adjust intervention strategies accordingly, in accordance with best practices and ethical guidelines.

  • Maintain accurate and timely documentation of all clinical interactions and interventions, adhering to departmental policies and regulatory standards.

  • Participate in interdisciplinary case conferences, team meetings, and continuing education activities to enhance clinical skills and stay abreast of developments in geriatric psychiatry, dementia care, and social work practice.

  • Performs other duties as required or assigned.

Qualifications

  • Master’s degree in social work from an accredited institution of higher education, including an internship meeting professional standards.

  • Must be able to demonstrate that he or she has provided psychological services, on an inpatient, outpatient or consultative basis in the past 12 months.

  • Licensed Independent Clinical Social Worker (LICSW) in New Hampshire
