Bent County Home Health Director of Clinical Services in Las Animas, Colorado

The Home Health Director of Clinical Services has primary responsibility for planning, implementing, and evaluating nursing and other skilled clinical services through systematic assessment of caseload and optimum utilization of personnel.

Level of Responsibility: Receives professional and administrative guidance from the Executive Director.

Supervises clinical field staff and related office support staff. Coordinates therapy and social work services.

Duties and Responsibilities: The following statements are illustrative of the duties and responsibilities of the job. Plans, organizes, coordinates, and directs ongoing clinical operations to ensure provision of adequate and appropriate care and services to include on call rotation. 2. Recruits, employs, and retains qualified personnel to maintain adequate staff. 3. Orients staff to agency policy and program, to services provided by other community agencies and organizations and to various third-party payer requirements. 4. Supervises clinical practice to always ensure a high quality of health care service and maintenance of agency standards. 5. Directs staff in the performance of their duties including admission, discharge, and provision of service to clients. 6. Provides services to clients to include admission, discharge, nurse aide visits, supervisory visits, etc., as needed. 7. Provides staff development through in-service educational programs, selected progressively responsible work assignments, individual guidance, performance evaluation, and other methods. 8. Stays current on local trends and issues and assures staff access to current clinical information and practices. 9. Participates in planning, implementing, and evaluating services. 10. Coordinates with other program areas and management as appropriate. 11. Assists in preparing annual budget with Director. 12. Monitors compliance with clinical services with budget and program goals. 13. Ensures compliance with regulations, accreditation standards and standards of practice. 14. Implements, integrates, and maintains Quality Improvement Program. 15. Establishes and maintains effective channels of communication and working relationships. 16. Attends and participates in appropriate client care conferences, staff and education meetings, and agency committee meetings. 17. Designate a similarly qualified alternate to be available at all times during operating hours in the absence of the Director of Clinical Services. 18. Assists Public Health Nurse with public health duties as needed.

  1. Provides first level investigation of complaints and critical incidents.
  2. Communicates with the physician and other providers on the care plan as necessary to assure appropriate and effective care.
  3. Maintains confidentiality in all aspects of job performance.
  4. Performs all job functions in compliance with all appropriate local, state, and federal laws, regulations and professional standards.
  5. Performs other related duties as assigned.

Knowledge, Skills, and Abilities: 1. Thorough knowledge of the practice of nursing. 2. Thorough knowledge of home health care. 3. Ability to recognize the needs and concerns of people including client interviewing and assessment skills and development of care plans and service agreements. 4. Knowledge of Medicare and other third-party requirements for reimbursement. 5. Ability to supervise effectively. 6. Ability to communicate effectively, orally and in writing. 7. Negotiation, intervention, and interpersonal communication skills. 8. Ability to interpret rules/regulations and convey them to administration. 9. Ability to accept and utilize administrative direction and consultation. 10. Knowledge of community resources. 11. Ability to work independently and exercise good judgment in problem solving and networking within the agency and community. 12. Interest in and appreciation of older adults and chronically ill individuals. Positive, pleasant, and understanding attitude. 13. Ability to establish and maintain effective working relationships with clients and families, staff members, and the general public.

Minimum Qualifications: Registered nurse who has at least two (2) years of professional nursing experience including one (1) year in home care or a closely related service.

Preferred Qualifications: Bachelor's Degree in nursing from an accredited institution.

Special Requirements: 1. Current state licensure to practice as a Registered Nurse. 2. Valid, current Colorado driver's license and proof of vehicle insurance,
