Lancaster General Health Ambulatory Pharmacist Clinician (Part-time/0.5FTE) in Lancaster, Pennsylvania
Summary
0.5 FTE ambulatory pharmacist clinician focusing on caring for patients with HIV or at risk for HIV infection. Five clinic sessions (clinic session=half day) per week with two sessions being evening STI clinics. The 3 other clinic sessions can be split during the week. Follows the LGHP holiday schedule. No call, no weekends. The pharmacist will provide medication therapy education for PrEP treatments, HIV treatments, as well as for STI risk management and treatments. In addition, the pharmacist may co-manage chronic conditions such as diabetes and assist the onsite providers.
Job Description
POSITION SUMMARY:
Provide clinical pharmacy services including pharmacist-managed clinics, transitions of care communication, and other direct patient care activities by designing, recommending, monitoring and evaluating patient specific pharmacotherapy. Serves as a clinical pharmacy expert for aspects of the ambulatory service line and is responsible for training and education as it relates to clinical activities, competency assessment, development and implementation of clinical programs, committee activities, etc.
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
Provide verbal and written consultations concerning drug therapy management upon referral or as necessary based on performance improvement opportunities.
Demonstrate ability to design, recommend, monitor, and evaluate patient-specific therapeutic regimens for ambulatory care patients, taking into account safe and cost-effective medication choices. Consider adverse drug effects, therapeutic goals and indications, contraindications/precautions, patient’s current status of health, drug-drug, drug-food, drug-laboratory and drug-disease state(s) potential interactions.
Evaluate appropriate laboratory tests and utilize physical assessment skills necessary to monitor and support the patient’s drug therapy program.
Demonstrate knowledge and communication skills to effectively counsel patients and answer questions.
Document assessment, observations, impressions, recommendations, plan, significant findings, interventions and services rendered as appropriate.
Identify drug therapy strategies that improve patient outcomes and reduce costs.
Active member of pertinent committees or workgroups and present on ambulatory pharmacy topics as requested.
Ability to assess the potential for new pharmacy roles in ambulatory practice and help justify them – requires creativity, ability to demonstrate impact of patient care services on outcomes.
Serve as a resource and mentor for pharmacy residents, students, and other allied health professionals concerning ambulatory pharmacy practice.
Serves as a role model of customer service by responding constructively to the needs of others. Responding constructively is defined as using a respectful communication style that is consistent with professional collaboration.
SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:
Provide accurate, unbiased, timely and cost-effective drug information, including educational drug therapy sessions for patient groups and health care professionals.
Evaluate and participate in drug research and other clinical trials, including designing and implementing primary research projects.
Collaborate with other health system leaders to address issues that impact readmissions or transition of care, such as medication reconciliation and quality initiatives.
Conduct drug utilization evaluations for ambulatory patient populations as appropriate to ensure safe, cost-effective quality care.
Interprets and evaluates all prescriptions and assures for safe and effective medication, dose route and scheduled administration with clarification as deemed necessary.
Other duties as assigned.
JOB REQUIREMENTS
MINIMUM REQUIRED QUALIFICATIONS:
Pharm.D. and ASHP-accredited Pharmacy Practice Residency required (or equivalent experience).
ASHP-accredited Specialty Residency or equivalent experience preferred.
Current licensure (or eligible to reciprocate) in state of Pennsylvania required.
Board Certification may be preferred (dependent upon specialty).
Equivalent of one year (1) experience practicing in an ambulatory setting.
A health care provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e. not excluded from participation in Medicare, Medicaid or any other federal or state health insurance program.
PREFERRED QUALIFICATIONS:
- Equivalent of three years (3) experience practicing in an ambulatory setting.
