Amergis Community Public Health Worker (disease investigator) in Lakeport, California

The Communicable Disease Investigator/Contact Tracer isresponsible for investigating situations concerning individuals who have beenin contact with communicable diseases, such as COVID 19 which could result inpossible sources of infection. The workincludes locating patients diagnosed with a communicable disease and personswith whom they have had contact. Investigation procedures involve contacting various information sourcessuch as welfare agencies, neighbors, hospitals, work locations, restaurants andbars. The Communicable DiseaseInvestigator/Contact Tracer is primarily concerned with locating persons whoshould be examined for acute infection. General supervision is received from an administrative superior withleeway allowed for the exercise of independent judgment and initiative incarrying out recommendations.

Essential Duties and Responsibilities:

  • Interviews patients to elicit information regarding theircontacts to positive patients and to determine possible personal and healthhistory related to transmitted diseases

  • Educates and counsels patients regarding transmitteddiseases and risk reduction

  • Locates persons who have been reported positive or who havebeen in contact with infected individuals; directs these individuals to submitnecessary laboratory specimens and such other necessary control measures

  • Induces persons who have been in contact with a communicabledisease to have medical examinations and laboratory testing as applicable

  • Provides information and education on communicable diseasesover the phone or in person

  • Conducts interviews with persons who test positive andassists them in informing their close contacts of their positive status

  • Conducts contact tracing and investigations to discoverunreported cases

  • Performs a variety of duties relating to the preparation ofreports, entering personal data on patient registers and preparingcorrespondence to send to patients who are in need of medical examination andtreatment

  • Maintains records on all delinquent and/or non-conformingpatients

  • Prepares individual narrative reports concerning workprocedures and certain patients

  • Utilizes appropriate methods for interacting effectively andprofessionally with persons of all ages and from diverse cultural,socioeconomic, educational, racial and ethnic backgrounds, sexual orientations,lifestyles and physical abilities

  • Performs other duties as assigned/necessary

Minimum Requirements:

  • High school diploma or equivalent

  • Medical experience, social work or public health experienceis preferred

  • Ability to show empathy to distressed individuals

  • Complies with all relevant professional standards ofpractice

  • Participation and completion of Amergis’ Competency programwhen applicable

  • Current CPR if applicable

  • TB questionnaire, PPD or chest x-ray ifapplicable

  • Current Health certificate (per contract or stateregulation)

  • Must meet all federal, state and local requirements

  • Successful completion of new hire training as applicable tojob site

  • Understand patient confidentiality and HIPAArequirements

  • Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors, co-workers,clients) via strong communication skills; proficiency in the English languageis required

  • Computer proficiency required

  • Must be at least 18 years of age
