WorkSource Oregon Behavioral Health Navigator, La Grande Family Medicine (5928) in LA GRANDE, Oregon

La Grande Family Medicine, of Praxis Medical Group, is seeking a motivated team-player to work as a Behavioral Health Navigator on our primary care behavioral health team. This is a full time position with an excellent benefits package. We\'re seeking an individual who works well with people and has a strong work ethic. The role of the BHC Care Navigator/Panel Care Coordinator is to assist the primary care behavioral care team (PCP, medical assistant, nurse, behavioral health provider) by coordinating care for patients on the provider\'s daily schedule and by proactively managing and coordinating care for patients who are part of the assigned panel. The BHC Care Navigator will act as a primary conduit for the transmission of information between providers and patients. They will coordinate services for all patients who are part of the assigned panel, especially those with serious, complex or chronic health problems or with psychosocial issues. They will help provide advocacy, information and referral services to patients and families to address their medical and psychosocial needs. Pay: Starts at \$18 per hour Schedule: Monday - Friday, 8:00 AM - 5:00 PM Education and Experience Required: - Experience or interest working with behavioral health preferred. - At least 2 years (90 credits) of college education or relevant training/certification. - Prior experience working in healthcare preferred. - Experience working with underserved populations preferred. - Communication skills, spoken and written, equivalent to 2 years of college education Knowledge, Skills and Abilities Required: - Proficiency in verbal and written communication in English - Demonstrated ability to work effectively in a team environment. - Demonstrated problem solving skills in a complex environment. - Demonstrated effective interpersonal relationship and customer service skills. - Good organizational and time management skills - Good working knowledge of local social service resources or skills to acquire and use this knowledge and information expeditiously. - Desire to work with diverse patients who have medical and behavioral health needs. - Ability to work effectively with people from diverse cultures and diverse socioeconomic situations. - Basic level of skill with Microsoft Word, Outlook, Excel and ability to use other computer programs and applications in ways that facilitate panel management (Electronic Health Record).
