The Cheshire Medical Center/Dartmouth Health Family Medicine Residency is seeking an experienced Advanced Practice Providers to join our growing team. The successful candidate will have a passion for comprehensive family medicine and team-based care with an interest in working in an educational environment.
In this non-paneled role, you will work closely with a small team of physicians and residents to collaboratively care for a patient panel and provide enhanced continuity. While under the general direction of the residency program director, you will practice autonomously and work closely with both faculty and resident physicians to provide high-quality team-based care for our patient population. For the ideal candidate, this is a unique opportunity to help shape our residency program and clinic that opened in October 2023 and welcomed our first class of six residents in June 2024. The practice strives to meet community needs, including care for marginalized individuals, and will have a special focus on care for LGBTQ+ populations. The position includes opportunities for a faculty role, leadership, quality improvement, teaching, professional growth and development, and a strong connection to community. Teaching experience, and experience with gender affirming care and substance use disorder treatment is preferred, but not required.

The Community Practice Environment
Cheshire Medical Center/Dartmouth Health Family Medicine Residencyis a three-year Family Medicine residency program based at Cheshire Medical Center in Keene, NH and affiliated with the Geisel School of Medicine at Dartmouth. Family & Community Care is the residency ambulatory Family Medicine practice, which is staffed by attending Family Medicine physicians, advanced practice providers and Family Medicine residents and provides primary care services for patients in the Monadnock Region. The program areas of focus include 1) comprehensive care for rural populations, 2) care for rural LGBTQ+ populations, 3) substance use disorder treatment, 4) population health and community medicine. Our mission is to train family physicians to serve the needs of patients, families and communities with compassionate, comprehensive, evidence-based, and patient-centered care, anchored in values of excellence, collaboration, growth, respect, diversity, and equity.
Cheshire Medical Center (Cheshire)is a 169-bed, Level III community hospital that has served the Monadnock Region of southwestern New Hampshire for over 125 years. The City of Keene (population 24,000)is a college town with a classic New England downtown with varied opportunities for shopping, dining, and an active arts community. The city has excellent public and private schools and extracurricular activities, along with a variety of housing options in the city of Keene and the small towns surrounding the city. Outdoor recreation opportunities abound, with mountains, lakes, forests, and ski resorts nearby, and Boston and New Hampshire’s Seacoast just a two-hour drive away.

Required Licensure/Certifications
Eligible for APRN or PA licensure in the State of New Hampshire.
Eligible for Federal DEA Registration.
