DODGE CO. HUMAN SERVICES & HEALTH Aging Disability Resource Specialist in JUNEAU, Wisconsin

JOB REQUIREMENTS: This position is responsible to assist elderly, persons with physical and or intellectual disabilities with information, referral, and linkage to long-term services, and follow up; to promote and preserve the autonomy, dignity, independence, and financial security of the consumer; and to provide responsive and courteous service to County residents. ESSENTIAL FUNCTIONS - Provides assistance through information, counseling, and referrals related to individual eligibility for public benefits and services, health care financing, insurance, housing, and other financial and consumer concerns. - Counsels on advantages, risks, and responsibilities related to long-term services and programs that may address individual needs. - Administers Department of Health Services (DHS)-approved functional screen to determine eligibility for long-term support programs. - Provides short-term case management. - Assists consumers in connecting with appropriate resources and services to meet their needs. Follow-up as needed to determine outcomes and to offer additional assistance. - Assists consumers with Medicaid applications, including gathering needed information and documents. - Provides choice counseling related to managed care. - Connects consumers in crisis to emergency services by providing short-term care coordination. - Provides outreach and information services to youth with disabilities/family caregivers as they transition into the adult long-term care system. - Links at-risk individuals with prevention and early intervention services. - Provides public education and/or training on issues affecting rights, benefits, or entitlements. - Develops informational material for the media, organizations, and the general public as requested. - Makes referrals to other agencies and services when appropriate. - Other duties as assigned. ***** OTHER EXPERIENCE AND QUALIFICATIONS: - Bachelor\'s degree in a health or human services-related field or a license to practice as a registered nurse in Wisconsin and one (1) year of relevant professional work experience with the aging, elderly, or persons with disabilities.
