We are committed to delivering exceptional care to children in our community-based homes and are seeking a dedicated RN Care Coordinator to join our ChildServe team. In this role, you will be responsible in providing both telephone and on-site support, ensuring that the healthcare needs of each child are thoroughly assessed, planned, and evaluated.     Schedule

  • On call Friday 4pm-Sunday 8pm, with 16-20 hours onsite during this time

    What You’ll Do

  • Provides healthcare delivery to children in community-based homes by being available both via telephone during assigned hours and being on site 16-20 hours per weekend.

  • Maintain regular communication with Primary RN Care Coordinators for purpose of coordinating healthcare management and ensuring continuity of care as well as reporting any adverse events or concerns with care delivery process to supervisor to ensure appropriate documentation and follow up.

  • Consistently communicates child’s healthcare needs to physicians, nurses, staff and families to assure each child’s needs are assessed, communicated and met.

  • Delegates tasks within scope of practice to Licensed Practical Nurses and Non-licensed caregivers, mentors and supports Resident Assistants, Certified Nursing Assistants and Certified Medication Aides to ensure delegated healthcare delivery aligns with ChildServe standards.

    What You’ll Need

  • Graduate of an accredited School of Nursing and licensed to practice as a Registered Nurse in the State of Iowa.

  • Nursing experience in the field of developmental disabilities, pediatrics, and behavioral health preferred.

  • Must have reliable transportation and be willing to become an authorized driver with ChildServe and use own car to transport children in the community.

  • Ability to lift up to 35 lbs, be mobile within the clinic areas, and frequently bend, lift, reach, and stand,

  • Active BLS from the American Heart Association or American Red Cross

  • Ability to communicate effectively in (or through) reading, writing, and speaking the English language
