Description
We are seeking a Full-Time Medical Social Worker in our Group Homes to provide social services to the families and their children or young adults in the Homes and support social service needs within ChildServe. Work as a member of the multi-disciplinary team to meet the psycho-social needs of the child or young adult and their family.
What You'll Do
Receives and processes referrals for Homes from parents, guardians and referring agencies.
Facilitates the admission process to ensure that all necessary documentation is obtained, reviewed and signed.
Educates and informs families regarding child/family rights and responsibilities and consent for services.
Assesses strengths and psychosocial, emotional and treatment needs of children/young adults and their families.
Collaborates with the multi-disciplinary team to ensure the care plan is developed and implemented as planned.
Identifies barriers for child/family to participate in their care or achieve care plan goals and assist in problem-solving through those barriers.
Ensure child/young adult’s access to Medicaid, private insurance and other associated programs/benefits
Serves as a primary liaison for the Department of Health and Human Services
Prepares child/young adult and their family for transition outside of ChildServe and develops a discharge plan with the care team that promotes continuity of care.
What You'll Need
Required: Bachelor’s degree in social work/human services
Preferred: Master’s degree in social work from an accredited School of Social work
Required: Three years of social work practice in a medical/healthcare setting
Preferred: Experience with the pediatric population in a multi-disciplinary skilled healthcare setting
Demonstrate proficient organizational and time management skills with developed problem-solving skills and decision-making, and ability to prioritize multiple demands.
Ability to communicate effectively and professionally with others, both verbally and in writing, and foster effective relationships with co-workers, children, and families.
Ability to interpret and follow applicable policies, procedures, and regulations.
Ability to communicate in English effectively for understanding, in writing and verbally.
