Summary The Social Work Department is hiring for a Social Worker Program Coordinator (Homemaker/Home Health Aide) Position. He/she is responsible for the timely payment to our vendor/ contacted agencies. He/she also oversees the use of respite services to veterans. The incumbent is delegated the responsibility for the planning and coordination of all elements which constitute the H/HHA program. This requires a high and varied level of professional expertise and collaboration to develop, revise and/or apply policies and standards which determine the scope of the Homemaker/ Home Health Aide and the duties of the program staff. The Coordinator will be responsible for ensuring the Veteran Care Agreements for H/HHA agencies are in good standing through consultation with the Care in the Community Office staff. The coordinator is supervised by the Non-Institutional Long Term Care (Nil TC) Social Work Supervisor. The Social Worker is capable of practicing at the advanced level and has specialized knowledge of practice typically related to the population served within the H/HHA programs. The incumbent is responsible for independently assuring that Central Office Geriatrics and Extended Care program standards and policies are adhered to in a timely and effective manner. The incumbent is administratively responsible for assuring that the Homemaker/Home Health Aide Program functions according to guidance in the Office of Integrated Veteran Care's Field Guidebook and VHA Notice 2024-01, Purchased Home and Community­-Based Services Programs. The Social Worker is capable of practicing at the advanced level and has specialized knowledge of practice typically related to the population served within the H/HHA programs. As the Program Coordinator, the incumbent is responsible for coordinating the work responsibilities of clinical and support staff assigned to the program. The Coordinator must ensure that the employees understand and adhere to the program directives, policies and standards as well as VAMC employee requirements. The incumbent is responsible for overseeing that payments to the H/HHA agencies are done in a timely manner and in compliance with VA standards. The incumbent is aware of CCN and VCA requirements for care and will report agencies that may not be practicing at a high level. The incumbent is also responsible for maintaining an accurate functional list of the various homemaker agencies. The incumbent will function as an active member of the Mountain Home GEC Committee. The incumbent is responsible for tracking pending consults, ensuring appropriate Standard Episodes of Care (SEOC) are entered using the Case Mix Tool (CMT), as well as tracking the hours of service per week provided and removing deceased or disenrolled Veterans from the program. The incumbent is responsible for participating in the Extended Care Leadership Committee, which includes presenting reports, action plans, budgets, and any other relevant information. The incumbent is the responsible party for overseeing the placement of veterans into the H/HHA program and assures that the placements meet VA eligibility requirements. The incumbent works is responsible for providing appropriate documentation, reviewing and follow-up of complaints regarding the H/HHA program. He/she is also responsible for organizing and preparing reports concerning the HHA program for the Medical Center Director, VISN liaison and Central Office. The incumbent provides both informal and formal education to VA staff on any changes in VA policy concerning Homemaker/Home Health Aide Program The incumbent attends and or participates in a variety of monthly and quarterly conference calls disseminating needed information to VA staff and community agency staff as warranted. Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Grade Determinations: Social Worker Program Coordinator GS 12 (1) Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
