Professional Responsibilities:

The incumbent(s) will report to Program Manager of the Office of Performance Improvement and will provide professional support with implementation and evaluation of Florida Department of Health in Duval County (DOH-Duval) programs and initiatives through local PHAB re-accreditation activities, agency Strategic Planning, Community Health Assessment and Community Health Improvement Planning, Workforce Development and Quality Improvement Planning. In alignment with local foundational plans, the incumbent(s) will provide local support to statewide initiatives such as: Florida Healthy Babies, Tobacco-Free Moms and Babies, Healthy Communities of Florida, Minority Health, and Age-friendly Florida. The incumbent will provide support the Healthy Jacksonville Coordinator with Community Health Improvement Plan implementation and the Healthy Jacksonville Collaborative partnership engagement activities. The incumbent may also provide direct support to the CDC-funded programs which may include theOverdose to Action Programas the well as theStrengthening U.S. Public Health Infrastructure, Workforce, and Data Systemsinitiative.

The incumbent should have a strong ability to foster team collaboration, support a common vision, and create momentum toward action and achievement of performance targets. The incumbent is required to possess strong organizational skills and ability to manage work plans to meet deliverable due dates. The incumbent is required to possess knowledge of evaluation techniques, health indicators, and public health principles to perform varied work assignments. The incumbent is expected to have subject matter knowledge and skill related to planning and implementation of performance improvement plans as well as cultivating local health department partnerships.

Competencies are the knowledge, skills, abilities, and behaviors that contribute to individual and organizational performance. In public health, successful mastery of competencies is vital to developing a strong workforce that can support community and program needs.

Work is primarily conducted within the offices of Florida Department of Health in Duval County. Tele-work is not available. Hours of operation are Monday through Friday from 8am to 5pm, excluding official state holidays and office closures.

Applicants should have strength in the following core competency domains:

Public Health Competency Domain 1: Analytica Assessment skills

Conducts data analysis to understand factors affecting the health needs of the community.

  • Develops data collection tools, surveys, and protocols.

  • Identifies qualitative and quantitative data that can be used for assessing the health of the community and the impact of performance improvement activities.

  • Provides analysis and participates in the development of community health reports that contribute to identification of strategic health priorities, health improvement goal setting, and creation and monitoring to improve health outcomes.

  • Effectively uses data to draw conclusions and make recommendation to inform decision making.

  • Conveys data and information to professionals and the public using a variety of approaches (e.g., written reports, oral presentations accompanied by PowerPoint presentations, factsheets).

    Public Health Competency Domain 3: Communication skills

    Collaborates with interdisciplinary teams to coordinate the development, implementation, and monitoring of the Community Health Assessment (CHA), Community Health Improvement Plan (CHIP) and local activities related to statewide initiatives such as Florida Healthy Babies, Tobacco-Free Moms and Babies, and Healthy Communities of Florida.

  • Constructively engages internal and external stakeholders to develop, implement, and monitor Duval County's CHA and CHIP cycles.

  • Revises Duval County's CHA and CHIP on annual basis based on stakeholder feedback.

  • Assists with the development of action plans to reach strategic objectives.

  • Ensures that healthy equity and access to care are cross-cutting strategies addressed through all CHIP priority areas.

  • Works effectively with stakeholders to coordinate quarterly and annual progress reporting.

  • Oversees implementation of local activities related to statewide initiatives such as Florida Healthy Babies, Tobacco Free Moms and Babies, and Healthy Communities of Florida with DOH-Duval and the community.

  • Coordinates development, implementation, monitoring, and reporting of action plans.

  • Participates in CHIP-related committees such as, but not limited to, the Minority Health Initiative, Tobacco-Free Jacksonville, etc.

    Public Health Competency Domain 6: Public Health Science skills

    Works collaboratively to develop and submit documents to meet PHAB Reaccreditation standards.

  • Maintains a knowledge base of evidence-based practices to inform the development of Duval County's CHIP, DOH-Duval's Strategic Plan, DOH-Duval's Performance Management & Quality Improvement Plan, and DOH-Duval's Workforce Development Plan.

  • Ensures alignment with the Agency Strategic Plan. the State Health Improvement Plan, DOH-Duval's Strategic Plan, and Duval County's CHIP, wherever possible.

  • Assists with the submission of documents for PHAB Re-accreditation, including the PHAB annual report.

  • Participates in Regional PMQI Consortia meetings and additional training opportunities related to PHAB Reacceleration.

  • Submits approved annual reports to the DOH Program Offices prior to annual deadlines and ensures reports meet DOH and PHAB Re-accreditation performance standards.

  • Performs miscellaneous job-related duties, as assigned.

    Emergency Duty: As an employee of the Florida Department of Health, the incumbent may be required to work before, during, and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in Special Needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

    Qualifications :

    Minimum:

    • Bachelor’s degree in public health, Community Health, Health Planning, Health Administration, Psychology, or a related field from an accredited college or university – or - at least 2 years of equivalent work experience.
  • 1+ year of experience communicating information in a professional manner (e.g., engaging community partners, presentations, public speaking).

  • 1+ year of experience writing reports containing technical information (e.g., action plans, health assessments, accreditation reporting).

    Preferred:

    • Master’s degree in public health, Community Health, Health Planning, Health Administration, Psychology, or a related field from an accredited college or university or equivalent work experience.
  • 1+ year of professional experience collecting and analyzing data.
