The Phoenix Center LPN in Ironton, Ohio

Job Duties:

· Reports to the Medical Supervisor

· Know and adhere to TPC's policies and procedures

· Coordinate with all other departments as needed

· Coordinate with Facilities Manager for all projects and medical needs

· Ensure compliance with all requirements associated with services, such as TB testing, medical assessments, and pharmacy orders

· Inspect equipment to ensure functionality

· Assist the medical provider with resident and staff medical care

· Ensure the department continuous compliance with all ACA, PREA, and departmental CBCF and BCS standards

· Ensure that all resident medications are provided in a timely manner and are verified for correct dosage and amount

· Review resident MAR to ensure compliance with medical orders

· Conduct periodic w Check email and voice messages to ensure up to the hour communications

· Coordinate with the Facilities Manager as needed, on all departmental related matters, transportation needs, and major projects.

· Review daily sick calls and medication counts

· Process new intakes and ensure files are completed within the allowed time frame

· Order and pick-up all prescription medications as needed

· Walkthrough of the facility to ensure compliance and cleanliness of managed areas.

· Provide guidance and corrective actions to TPC residents as needed

· Maintain relationships with vendors, inspectors, and outside entities

· Weekly meeting with Facilities Manager to discuss upcoming projects and expenses

· Conduct weekly medication counts and process orders for refills

· Request and administer Vivitrol lab work and injections to graduates

· Assist the medical provider with resident care

· Review and verify that medications other supplies are adequately stocked and organized

· Conduct visual inspections of medical area to ensure all equipment is in working order and good repair

· Conduct resident hygiene class

· Prepare new MAR and file old MAR in resident medical files

· Resident hygiene class

· Check first aid kits throughout facility

· Conduct quarterly quality assurance audits/reviews for all departments under supervision to ensure compliance with ACA, PREA, CBCF and BCS standards

· Attend scheduled trainings to continue professional development

· Work with the County Health Department for HIV, Hepatitis, and communicable disease training and testing.

· Ensure biohazard materials are properly disposed of

· Review Departmental Standard Operating Procedures (SOPs) and relevant agency policies; recommend changes as necessary

· Assist the Operations Director in collection, review, and approval of annual ACA audit documentation

· Review the Code of Ethics and recommend changes to the Executive Director

· Review training records to ensure training goals have been met and documented for all supervised staff

· Provide and track TB testing for all staff

· Work with the local County Health Department for annual flu vaccines for staff and residents

· Inspect facility 1st Aid kits and AED devices

Minimum Qualifications:

· LPN licensure

· The applicant must show proof of holding and must continue to maintain valid permits, licenses and certifications as may be required for this.

· Valid Driver's License

Education Requirements:
