The Anticoagulation Clinic is an outpatient center that provides support to patients facing cardiovascular disease. At the clinic, we help cardiovascular patients manage their blood thinner medication therapy. The Anti-Coag Clinic RN is responsible for maintaining patients' INR values within the ordered range by adjusting anti-coagulation medication according to the provider established protocol. The Anti-Coag Clinic RN documents results, dosing and any other pertinent information affecting the patient’s anti-coagulation therapy and determines appropriate medication dosage for patients based on lab results by utilizing physician established protocols in conjunction with nursing judgment as needed. This position conducts patient teaching regarding anti-coagulation therapy, medication changes, and the dates of next office visits or lab appointments. The Anti-Coag RN monitors for patients who have missed Anti-coagulation Clinic appointments or who are overdue. This position refills patients' oral anti-coagulation medications according to protocol, communicates patient changes or problems to providers, and maintains patient confidentiality while adhering to HIPAA regulations.
Your exceptional qualifications
Graduate of a NLN, ACEN or CCNE accredited nursing program, or three years related professional nursing experience.
Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).
Minimum of 2 years of nursing experience
