Job Description
We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The Pharmacist will evaluate physician medication orders, maintain patient databases, ensure the appropriate ordering of laboratory tests, and collaborate with the healthcare team to provide clinical and technical support. This role also involves implementing policies for the safe use of drugs and maintaining necessary documentation in compliance with federal and state regulations.
Responsibilities
Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
Maintain patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.
Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
Collaborate with the healthcare team to provide clinical and technical support as needed.
Document all communication with physicians, nurses, other healthcare team members, patients/residents, and their family members in the medical record.
Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as the disposal of drugs.
Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and company policy.
Inform the Pharmacy Manager of medication-related errors.
Assist facility/agency staff with procedures related to the safe and effective procurement, maintenance, administration, and disposal of drugs.

Pay and Benefits
The pay range for this position is $63.00 - $63.00
