Family Continuity is rapidly expanding its energetic workforce to lead fast paced, innovative clinical programming in our six different sites throughout Northeast, Southeast & Central Massachusetts. The Family Resource Center in Hyannis, Massachusetts is seeking a Family Partner to use the knowledge gained from life experience to empower, educate, & support families who have a child with mental health issues. The Family Partner is responsible for the establishment & maintenance of relationships between treaters & family members of persons served by Advocates. When these relationships are aligned, people in the services of Advocates are in the best conceivable circumstances to acquire & generalize skills necessary to their long-term success. This position operates as a bridge between families seeking knowledge of & input into their loved ones? lives & the work of those striving to make those lives truly worthy of pursuit. Minimum Education Required High School Diploma/GED Additional Shift Details this position will require some evening & weekend hours Responsibilities Form meaningful positive relationships with individuals & families served through the agency Work in conjunction with the Clinician to develop the Family Support Plan for family members experiencing CRA-related issues. Oversee the implementation of the Family Support Plan with support from other FRC staff. Ensure youth & family voice & choice is implemented in service delivery. Facilitate child & family participation in the development & understanding of the care plan goals & interventions. Provide or support activities that are designed to address one or more goals on the youth?s care treatment plan. Completion of progress notes, ancillary individual & family documentation in record & submission of all billing. Complete incident reporting & any documentation as required by supervisor. Responsible for upkeep & organization of individual & family record. Qualifications High School Diploma or GED Must have at least one (1) year of experience working with the target population, or navigating any of the child & family servings systems & teaching family members who are involved with the child & family serving systems Or have lived experience with \"child helping systems.\" For example, navigating IEP\'s, mental health, court involvement, etc. Current driver\'s license, proper auto insurance coverage, reliable transportation, & ability to travel within the community Must become First Aid/ CPR certified if not currently.
