State of Massachusetts Community Mental Health Worker II in Hyannis, Massachusetts
The Cape Cod & Islands Community Mental Health Center, operated by the Department of Mental Health, is seeking to fill a Community Mental Health Worker position (MHW II) in the Adult Community Clinical Services (ACCS) program. This position acts primarily as an advocate for an assigned caseload of clients in the ACCS program, assisting individuals with meeting their rehabilitation goals. The selected candidate will participate in the development of comprehensive assessments including the development and implementation of assigned clients' Individual Action Plans. Additional responsibilities include securing appropriate services in community such as SSI/SSDI, programs for day structure, psychiatric and medical services. The incumbent in this position is expected to maintain a therapeutic atmosphere through professional performance and attitude to promote quality care and person served growth. Transporting clients to and from appointments or activities, using a program vehicle or their own vehicle, is required. _Please Note:_ * This is a full-time position with a work schedule of 40 hours per week on first shift. The work schedule is 8:30am to 5:00pm. The days off are Sunday and Monday. *Employees in this position will be required to work mandatory overtime. _Duties and Responsibilities (these duties and responsibilities are not all inclusive):__ * Engage persons, individually and/or in group, in the development of meaningful activities, such as recreational, educational, health and wellness activities within the program and the community to foster an improved quality of life. * Provide supportive counseling and maintaining regular contact with persons served and families via outreach in their home or other community environments. * Interpret and communicate all pertinent information, attending team and staff meetings and informing supervisor or designee of any critical information. * Involve persons served and all identified natural supports and collateral providers in the process of treatment planning. * Maintain person served records in a timely manner by documenting in EMR and paper record and completing all other related forms to ensure compliance with DMH regulations and Rehab Option requirements. * Provide culturally and linguistically competent services that are also sensitive and responsive to religious practice, sexual orientation, gender differences, physical disabilities, as well as services that are age and developmentally appropriate, including young adults and elders. * Foster a professional, courteous and respectful environment in dealing with persons served, families, coworkers and other service providers. *Required Qualifications:*_ * Ability to follow written and oral instructions. * Capacity to perform basic mathematics. * Proven ability to gather information through observing and questioning individuals. * Talent for communicating effectively in oral expression, writing clearly and concisely, and developing ideas in logical sequence. * Skilled in interacting with people who are under emotional stress with the ability to maintain a calm manner in stressful or emergency situations. _Preferred Qualifications:__ * Experience in and/or knowledge of human services. * MAP Certification. * Experience with DMH regulations and program procedures. * Familiarity and implementation of MAP policies. * Experience working with individuals with serious, persistent mental illness, substance abuse and/or who have experienced homelessness. * Given the population served, bilingual or multilingual fluency. *Special Requirements: * _ * Please note in accordance with an agreement between the Commonwealth of Massachusetts, Department of Mental Health and AFSCME/Council 93, employees assigned to a work unit where the Medication Administration Program (MAP) is used are required to pass MAP certification within 6 months of entering the position or 3 examination opportunities, whichever comes first. Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines.
