Crestwood Medical Center is currently seeking to expand its access to care in search of an

Integrated GME Behavioral Health Professional (LCSW) for its future ACGME-accredited Family Medicine Residency Program and associated Family Medicine Practice clinic (FMP).This position will become a part of the family medicine residency teaching and patient care team within the FMP.

The LCSW will identify, triage, and manage patients with medical and behavioral health conditions within the FMP. They will also provide skills training through psycho-education strategies and will provide brief interventions that are problem-focused and based on goals set in partnership with the patient. The LCSW applies specialized clinical knowledge and advanced clinical skills in the areas of prevention, assessment and treatment of mental, emotional, behavioral and addictive disorders; engages in multi-disciplinary treatment planning and clinical social work interventions. The LCSW will lead case management and work with the residency program director to engage faculty, residents, and community members on the clinic’s advisory council in care and regarding needs for additional supportive services. From time-to-time and at the direction of the residency program director, the LCSW will lead some lectures, small group discussions, Balint groups, or journal club for residents during the residency’s academic half-days.
