Job Description: * We are seeking a compassionate and dedicated individual to join our team as a Community Mental Health Home Health Aide. In this role, you will provide essential support and assistance to individuals with mental health needs within their homes and community settings. This position offers an opportunity to make a meaningful difference in the lives of others while working in a dynamic and rewarding environment. * Responsibilities:

  • Assist clients with activities of daily living, including personal hygiene, grooming, dressing, and meal preparation.
  • Provide emotional support and companionship to clients, fostering a supportive and nurturing environment.
  • Monitor clients' mental and physical well-being, reporting any changes or concerns to the appropriate healthcare professionals.
  • Implement and follow individualized care plans developed by mental health professionals, ensuring that clients receive comprehensive and personalized care.
  • Accompany clients to appointments, outings, and recreational activities, promoting social engagement and community integration.
  • Document observations, interventions, and progress notes accurately and timely in accordance with agency policies and procedures.
  • Collaborate with interdisciplinary team members, including nurses, therapists, case managers, and family members, to coordinate care and support client goals.

Requirements:

  • High school diploma or equivalent.
  • Previous experience working in a mental health setting or with individuals with mental health challenges is preferred.
  • Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) is desirable.
  • Compassionate and empathetic attitude with a genuine desire to help others.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and respect the privacy of clients.
  • Reliable transportation and valid driver's license.
  • Ability to pass background checks and drug screening.
  • Willingness to undergo additional training and professional development as required.
