Always Best Care Senior Services - Oahu is currently seeking a highly skilled and dedicated Physical Therapist to join our esteemed team. This exceptional opportunity allows you to make a positive impact on the lives of our valued patients in the beautiful city of Honolulu, Hawaii. We are looking for a compassionate and experienced Physical Therapist who will provide exceptional and personalized care to our patients in the comfort and familiarity of their own homes. This role offers flexibility, allowing you to create your own schedule according to your personal needs and preferences. Responsibilities: * Conduct comprehensive assessments to determine the unique needs of each patient. * Collaborate with physicians to develop and modify personalized treatment plans based on patient progress. * Contribute to the development and implementation of interdisciplinary patient care plans specifically tailored to physical therapy. * Administer diagnostic and prognostic tests to evaluate muscles, nerves, joints, and functional abilities. * Utilize advanced physical therapy techniques to effectively alleviate pain, enhance function, and optimize performance. * Educate and guide patients and their families in their physical therapy program. * Coordinate outpatient services for procedures that cannot be conducted at home. * Maintain accurate and timely records of evaluations, treatments, and therapy intervention responses. * Provide supervision and support to Physical Therapy Assistants and paraprofessionals. Requirements: * Graduate of a Physical Therapy school approved by the Council of Medical Education of the American Medical Association, in collaboration with the American Physical Therapy Association. * Hold a valid license to practice Physical Therapy in the state of Hawaii. * Minimum of one year of institutional/home health experience (preferred). * Successfully complete a comprehensive criminal background check. * Current CPR certification.
