What you'll do The primary purpose of the Care Coordinator, Registered Nurse Nursing Home is to assist the Director of Nursing in directing and supervising Nursing Services concerned with resident care; Responsible for the completing all job duties of a Nurse Care Coordinator (in her/his absence); Evaluate and direct nursing care of residents and is constantly available to the nurses in charge of the various units; Must be aware of changes in condition of residents and the indicated action. Integrate the philosophy and objectives of the Nursing Home and Nursing Department into the management of nursing care on his/her units in the nursing home. Participate in nursing care committees as recommended by the Director of Nursing. Maintain standards of care for each resident in the nursing home: Initiate the admission assessment of the resident gathering information to identify the needs and plan of care of the resident. Is responsible for initiating, updating, and reviewing the comprehensive care plans as it relates to the nursing service and the presentation of the same to the multi-disciplinary team. Correlate the comprehensive care plan with that of the nursing care plan for each resident on unit. Correlate the comprehensive care plan with the medical plan of care as determined by physician orders and conferences with the attending physician. Interpret care to residents, families, nursing staff, and other nursing home personnel. Participate in resident-teaching programs. Participate in writing care plans and defining policies and procedures. Discuss administrative and nursing care problems with other Nursing Supervisors and Director of Nursing. Determine needed nursing care by assessing residents, charts, care plans and by communicating with the staff and physicians on a daily basis, as needed. Assessment of needs, identification of nursing diagnosis based on histories, nursing problems and solutions. Work closely with the In-Service Coordinator to provide programs suitable to the care of the residents on his/her unit(s). Keep informed of new medical and nursing measures as related to the needs of residents. Maintain an environment conducive to providing quality care. Make rounds to observe effectiveness of care and institute immediate changes as necessary. Cooperate with attending physicians during their visits. Give care to residents who require nursing judgment and skill. Demonstrate new nursing techniques approved by the facility. Maintain a system for communication of nursing care information: Participate in nursing care committees. Complete nursing documentation in accordance with facility policies and procedures. Completes all required Medicare documentation. Report nursing care information to other Nursing Supervisors and Director of Nursing. Use nursing care plans as a guide for reports, rounds and conferences. Share information with other professional disciplines, facility departments and community agencies. Maintain an effective communication system: Review critical incidents and discuss with Director of Nursing and other Supervisors and interpret to staff. Suggest areas of study and participate in nursing research. Complete incident reports, review reports with Nursing Supervisor/DON. Assign personnel to nursing units based on needs of residents and professional or technical competence of workers. Identify problems of nursing care and assist team in assignment of personnel. Report staffing needs to the Director of Nursing. Assist with writing periodic evaluations and discussing individual goals with the supervisors and charge nurses. Supervise and counsel personnel assigned to his/her unit. Create and maintain a working climate conducive to producing quality care. Respect individual for ability, knowledge and skill. Plan opportunities for growth development. Support personnel policies; Keep personnel informed of changes. Recommend changes to the Director of Nursing. Assume the responsibility of the staffing of her/his respective unit(s). Assume responsibility for teaching relative to planning, implementing and evaluating nursing care. Participate in the orienting programs. Participate in any In-Service programs. Guide working relationships within unit with residents, families, visitors, nursing personnel, medical staff, other departments in facility and representatives of community agencies. Insure each resident is treated with dignity and respect and report any concerns with resident care and treatment to Administration in accordance with facility policy. Must follow the established infection control procedures. Participate in In-service education programs and in continuing education programs designed to keep abreast of changes in the nursing profession, keep RN license current and provide a copy for personnel file. Adheres to facility Code of Conduct including a commitment to professionalism and service excellence. Any other duties as assigned. Attend all required in-services and follow the department and facility policies including the use of personal protective equipment. Maintain CONFIDENTIALITY of all pertinent resident care and employee information and follow HIPAA and HIV regulations. What we need from you Education: Graduate of an accredited school of nursing, required All RN’s hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor’s prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN’s licensed on or before 12/19/17 are/remain exempt from this requirement. Experience: Minimum one year experience as general staff nurse, preferred Minimum two years experience nursing supervision, preferred License/ Certification: Current NYS licensure as a Registered Professional Nurse, required Valid, NYS Driver's License and the ability to travel, required Skills: Leadership Communication Gathering/Extracting Information Computer Skills Creative Thinking Physical Requirements: Sedentary Work - Exerting up to 10 lbs of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. This position is eligible for a sign on bonus up to: USD $35,000.00 Pay Transparency Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Pay Range Minimum: USD $97,760.00/Yr. Pay Range Maximum: USD $99,528.00/Yr.
