This position is responsible for collaborating with the Program Manager on creating a welcoming atmosphere for incoming clients, monitoring and preparing them for continued treatment in the broader system of care.  Position will perform intakes/admissions, treatment plans, monitoring of client’s progress, maintaining documentation per organization’s guidelines, discharges and resources to assist the client in finding appropriate ongoing services as well as oversee the activities of other staff and provide guidance when needed.

  • Accountabilities and Responsibilities:

  • Perform client admissions, transfers, assessments, referrals, and discharges daily

  • Communicate daily with Clinical Supervisor and Program Manager to ensure services are being delivered in alignment with department policy

  • Maintain a positive, welcoming approach with all incoming and present clients at the center

  • Provide ASAM screening and assessment at intake

  • Ensure daily census requirements are being met as directed by manager by ensuring that appointments are scheduled quickly and with minimal delay from treatment request to first appointment

  • Perform initial screening/assessment, recovery and treatment planning/problem list and individual check-in with clients to ensure goals are being achieved

  • Reconcile daily logs, rosters, and other census information to ensure daily census accuracy

  • Develop initial ongoing ASAM-informed referral and discharge planning with clients

  • Meet weekly for group clinical supervision

  • Inputs data into HSI’s Electronic Health Record System (EHR) and/or the EHR platform of the County funder

  • Maintain appropriate records in the EHR

  • Daily monitoring and maintenance of client EHR for errors, correcting and updating

  • Develop and maintain positive working relationships with all staff members and community organizations

  • Coordinate with program Case Manager to ensure referral mechanisms are properly engaged to maximize post-discharge treatment

  • Maintain attendance and punctuality to perform duties as scheduled

  • Attend all staff meetings and required agency meetings unless excused

  • Responsible for always maintaining a clean environment

  • Responsible for informing Program Manager of perceived household, environmental, participant, and personal needs when appropriate

  • Maintain a basic knowledge of medications used by clients

  • Drive program vehicle when necessary

  • Confer with staff and other individuals in agency regarding participants

  • Perform any other duties as assigned

  • Performs vitals sign patient self-monitoring tasks as needed to ensure clients fall within established safe stabilization guidelines, if applicable at program site.

  • Performs withdrawal assessment scoring duties to ensure clients fall within established safe stabilization guidelines, if applicable at program site.

  • Responsible for supervising meal preparation and, when necessary, preparing and serving meals

    Adheres to confidentiality requirements as governed by HIPAA and 42 CFR

    What else is required?

  • Registration with either Substance Use state certifying body in California (CAADE/CCAPP/CADTP)

  • Demonstrated experience working with high and at-risk population.

  • Valid California driver’s license with a good driving record and car insurance.

  • Current CPR/First Aid certificate desirable .

  • Background check and drug testing required at time of hire.

  • TB test annually

  • Current flu vaccine/flu shot.

  • The California Department of Public Health has required that all staff in healthcare organization receive the COVID 19 vaccination and booster up to date. Our organization is a healthcare organization and will abide by this mandate.

    Salary: $25.00- $30.64 per hour,  based on experience and qualifications.
