How can you make a difference in people’s lives?

The case manager is responsible for case management and referral planning for individuals that attend the Drop-In Center to help them find resources and community support that bring them into positive and healthy environments and opportunities. Duties include care coordination, maintaining positive working relationships with outside community providers, scheduling appointments, arranging transportation, assisting with ongoing care and discharge planning. The case manager will collaborate frequently with the Program Manager, Youth Peer Specialist, Mental Health Specialist and Interns to ensure clients basic needs are being met.

Responsibilities:

  • • Provide case management services to clients at the drop-in center. 

  • Schedule and arrange client medical, psychiatric, and or continuing care appointments. 

  • Maintain appropriate notation and documentation in client file and EHR. 

  • Maintain accurate documentation in accordance with case management billing practices. 

  • Provide timely crisis intervention when needed. 

  • Meet with all clients while at Drop-in center to determine case management and referral needs.  

  • Participate in all required consultation with Alameda County and Behavioral Health providers regarding client care and case management experience and opportunities.  

  • Maintain positive working relationships with community providers and managed care organizations.  

  • Assist with case management training at staff meetings. 

  • Provide clients with educational materials and orientation of continuing care options, mental health services, residential treatment, educational support, housing support and employment support. 

  • Demonstrate good decision making on a daily basis. 

  • Utilize email and other forms of technology to communicate with peers, supervisors, and community members. 

  • Attend all scheduled team meetings and trainings.  

  • Assist with data entry in to EHR system as needed.  

  • Drive program vehicle when necessary. 

  • Perform any other duties as assigned by Program Manager.

  • Excellent computer skills, knowledge of data capture and data input into Electronic Health Records.

  • Ability to communicate using email and other electronic systems. 

  • Familiarity with Substance Use and Mental Health programs, hospitals, and other continuing care options

  • Experience working with customers or clients that feel overwhelmed or in a crisis (crisis intervention).

  • Experience working with clients who have substance use disorders.

  • Extensive experience with case management of underserved populations in a public setting.

  • Experience working with diverse populations and cultures.

  • Experience working with customers or clients with co-occurring disorders and disabilities.

  • Familiar with customer service principles.

  • Experience with resolution of conflict in stressful situations.

    What else is required?

  • Associate or Bachelor’s Degree in Social Work, Psychology or related field, preferred. 

  • Experience providing case management services in a mental health or substance use disorder program in the non-profit sector.  

  • Experience serving youth and the LGBTQIA+ population. 

    Compensation and Benefits

    Salary:   $61,568 Annually ,based on experience and qualifications. Available for a full-time position.
