The Community Care Clinic (CCC) is located at 132 Jefferson St on the third floor of the Hartford Hospital Community Health building. CCC clinic has close to 4,000 patient visits annually with an average of 70 patients per day. The Division of Infectious Diseases provides inpatient and outpatient consultation regarding the diagnosis and management of all types of infectious diseases. The service is supported by outstanding clinical diagnostics laboratories, which provide state-of-the-art techniques for rapid diagnosis of infectious diseases. Our staff of providers, Psychiatry, fellows, Psych Residents, Social worker, Nutritionist, Pharmacy Liaison, APRNs, RNs, MAs, Case ManagerS and a Data Manager who provides compassionate care, excellence in teaching and investigations in clinical and laboratory research. CCC is Ryan White funded. 75% are bilingual with Spanish being their primary language. 80% of our patients have Health coverage under Medicaid. Our specialists are skilled at treating many infectious conditions, including: Conditions such as HIV infection, Hepatitis, fever of unknown origin, recurrent infections or rashes of unknown type or origin, Influenza, Opportunistic infections in patients who are immunosuppressed due to acquired or congenital immunodeficiency, transplant or other medical condition. CCC guides patients through the health system, including appropriate referrals for services to other health professionals. Job Summary: The role is responsible for the organization and coordination of data collection tools, acquisition of data for metric based programs. Provides day-to-day operational, technical and problem-solving on administrative matters including, but not limited to: • Planned systems as they relate to the integration and management of patient data, review for accuracy, analyze report data, support quality efforts, develop and implement mechanisms to track annual performance measures for grant funded programs • Review data entry into CAREWare, Evaluation Web, state or federal database, and health record to ensure accuracy, perform data entry when needed. • Produce periodic reports • Create and Present quality dashboards to the Ryan White team both locally and statewide. • Present ideas effectively, both orally and in writing for local, state or federal platforms • Design and maintain summary reports for staff use and/or for federal and state funders • Work effectively with funded providers local, state and federal • Travel to learning sessions as required • Other duties as assigned • Reports to assigned supervisor This is a grant funded position. • Bachelor’s Degree • Must be capable and willing to cover administrative front office staff depending on the department’s needs: scheduling appointments, answering telephones, signing in and discharging patients, copying and faxing • Knowledge of computer systems: Microsoft Office, EPIC. • Knowledge of medical coding preferred i.e.: CPT and ICD10 coding • Excellent communication skills. • Work effectively with funded providers and balance work between multiple projects • This position is located within a busy outpatient ambulatory care clinic setting.
